妊娠早中期超声对预测持续性前置胎盘的研究进展

摘要： 妊娠早中期前置状态的胎盘绝大多数在妊娠中后期迁移至正常位置，只有少数妊娠晚期和（或）分娩期胎盘仍然为前置状态，这部分可称为持续性前置胎盘（persistent placenta previa，PPP）。妊娠中期有多项超声指标：如胎盘下缘与宫颈内口之间的距离、胎盘下缘厚度、胎盘附着部位和特异性超声图像等，均可用于预测PPP，其中胎盘下缘与宫颈内口之间距离的测量与预测最为方便，准确可行。总体而言，单项指标对PPP的预测价值有限，预测价值不高，目前尚缺乏结合多项指标来预测PPP的模型。针对单一超声指标对PPP的预测价值进行阐述，并对建立PPP的预测模型进行展望。

Abstract: Many cases of placenta previa diagnosed during the early second-trimester have migrated toward the uterine fundus，but a few of them are still present at third-trimester and/or delivery. We name it as persistent placenta previa（PPP）. Lots of index can used to predict PPP in second-trimester，such as：os-placenta distance，the low placental edge thickness，placenta localization，the specificity of ultrasound images，etc. But in these index，the os-placenta distance is the most convenient，accurate and feasible. Over all，the single index have limited value in predicting PPP，but the prediction is not much valuable. At present，the model which is combined with a number of indicators to predict PPP is insufficient. In this paper，we assess the predict value of PPP with a single index by ultrasonography，and prospect for the establishment of prediction model with multinomial index for predicting PPP.
