揿针联合硬膜外分娩镇痛对自然分娩疼痛影响的研究进展

摘要:

分娩痛是产妇分娩必须面临的痛苦经历,对其生理与心理可造成多重不良影响。硬膜外分娩镇痛虽已成为临床主流镇痛方式,但其存在镇痛效果个体差异大、可能影响产程进展及药物相关不良反应等局限,临床应用有一定限制。揿针作为中医特色疗法,具有操作简便、安全、无不良反应等优势,已广泛应用于各类疼痛治疗且疗效确切。通过系统梳理揿针联合硬膜外分娩镇痛的相关研究后发现,该联合方案对自然分娩产妇的镇痛效果显著优于单一镇痛方式,可有效改善产妇分娩体验,优化母婴结局,具有较高临床推广价值。但当前揿针联合硬膜外分娩镇痛的相关研究仍存在诸多问题,如临床样本量偏小、多中心研究匮乏、分子机制基础研究尚不充分,且临床应用缺乏统一规范等。未来需着眼于上述问题,为揿针在分娩镇痛中的应用提供更科学的依据,提升揿针的临床疗效,加强揿针在产科领域的应用。

Abstract:

Labor pain is a distressing experience that parturients inevitably face during childbirth, exerting multiple adverse effects on their physiological and psychological well-being. Although epidural labor analgesia has become a mainstream clinical analgesic method, it is associated with limitations such as significant individual variability in analgesic efficacy, potential impacts on labor progression, and drug-related adverse effects, which impose certain restrictions on its clinical application. Press needle, as a characteristic therapy of Traditional Chinese Medicine, offers advantages including simple operation, safety, and absence of adverse reactions. It has been widely applied in the treatment of various pain conditions with confirmed efficacy. A systematic review of studies on press needle combined with epidural labor analgesia reveals that this combined regimen provides significantly superior analgesic effects for parturients undergoing natural childbirth compared to single analgesic modalities. It can effectively improve the maternal childbirth experience, optimize maternal and neonatal outcomes, and demonstrates high clinical value for broader application. However, current research on press needle combined with epidural analgesia still faces several issues, such as small clinical sample sizes, a scarcity of multicenter studies, insufficient fundamental research on molecular mechanisms, and a lack of unified clinical application standards. Future efforts should focus on addressing these aforementioned problems to provide a more scientific basis for the application of press needle in labor analgesia, enhance its clinical efficacy, and strengthen its utilization in the field of obstetrics.
