Research Progress on the Correlation between Human Papilloma Virus and Vaginal Intraepithelial Neoplasia/Vaginal Cancer

摘要:

人乳头瘤病毒(human papilloma virus,HPV)感染是女性下生殖道病变的重要致病因素,其中高危型HPV持续感染与阴道上皮内瘤变(vaginal intraepithelial neoplasia,VaIN)及阴道癌,尤其是阴道鳞癌的发生、发展密切相关。由于VaIN缺乏典型的临床症状,隐匿性显著,VaIN2/3是临床诊疗中易漏诊的阴道癌前病变。近年来随着宫颈癌筛查中HPV检测与阴道镜检查的规范化普及,早期阴道癌与VaIN检出率也显著升高。系统梳理近年来HPV与VaIN及阴道癌相关性的临床研究进展,以期为该类病变的临床防治策略制定提供参考依据。

Abstract:

Human papilloma virus (HPV) infection is a significant causative factor for lower female genital tract lesions. Persistent infection with high-risk HPV types is closely associated with the development and progression of vaginal intraepithelial neoplasia (VaIN) and vaginal cancer, particularly vaginal squamous cell carcinoma. As VaIN lacks typical clinical symptoms and is highly occult, VaIN2/3 represents a vaginal precancerous lesion prone to missed diagnosis in clinical practice. In recent years, with the standardized application of HPV testing and colposcopy in cervical cancer screening, the detection rates of early-stage vaginal cancer and VaIN have also significantly increased. This review systematically outlines recent clinical research progress on the correlation between HPV and VaIN as well as vaginal cancer, aiming to provide a reference for formulating clinical prevention and management strategies for these lesions.
