Abstract: Endometriosis，endometriotic cyst，uterine fibroids， endometrial polyps，septum and intrauterine adhesions were associated with female infertility. Surgery treatment could improve postoperative pregnancy rate and decrease recurrence rate compared with drug therapy. Reproductive surgery played an important part in improving IVF outcomes and fertile ability. Ovary endometriotic cyst would not affect the pregnancy rate of IVF， but it′s efficient for improving reproductive ability and decreasing pain by surgery. Adhesions and ovary damage should be prevented in the surgery. Endometrium and intrauterine diseases could affect IVF outcomes. Hysteroscopy examination should be used when IVF failed more than once to correct intrauterine diseases. Gynecological operation provided to women in reproductive age should follow the fundemental of microdamage and reproductive ability protection.
