The Effect of Combined-epidural Labor Analgesia on Stages of Labor Intrapartum Endocrine and Neonate Blood Sugar in Gestational Diabetes Mellitus

Abstract: Objective:To evaluate the effect of combined-epidural labor analgesia(CSEA) on stages of labor intrapartum blood sugar, serum cortisol and neonate blood sugar in women with gestational diabetes mellitus(GDM). Methods:48 cases of primiparas diagnosed GDM received CSEA, and were observed as studied group and the other 45 cases without receiving CSEA were observed as control group. All the stages of labor ,intrapartum blood sugar, serum cortisol, cortisol in umbilical cord blood and neonate blood sugar were determined. Results:①The blood sugar in studied group were significantly lower than the control in the first、 second and third hours during active phase(P<0.05) and the blood sugar in studied group is ralatively stable . The serum cortisol of the pregnant in studied group were significantly lower than the control in end of the active phase(P<0.05). ②The cortisol in umbilical cord blood in studied group were significantly lower than the control(P<0.05). The blood sugar of neonate in studied group were significantly higher than the control(P<0.05). ③The time of the active phase in studied group were significantly shorter than the control(P<0.05). Conclusions:CSEA maybe could shorten the time of the active phase and effectively eliminate the fluctuation of blood sugar and serum cortisol in gestational diabetes mellitus, and also improve blood sugar of neonate.
