Prediction of Preeclampsia in the First Trimester
    CHEN Xu;CHANG Ying
    2013, 40 (6):  489-491.
    Abstract ( 1259 )   PDF (392KB) ( 4760 )
Preeclampsia is a obstetrical complication with high perinatal mortality and morbidity. It is characterized by shallow trophoblast invasion in maternal uterine spiral arteries. Despite there are more and more understanding on the pathophysiology of preeclampsia,it is difficult to predict effectively in time in clinic. There are some relationship between the disordered placenta function and the poor pregnancy outcomes. However, any single parameter of placenta function could not be used as a exact predictor for preeclampsia. The improved ultrasound will provide a useful novel tool for prediction, but measurement of ultrasound need to be standardized. The test of biochemical markers in the first trimester, combined with the uterine artery Doppler screening, may be a promising screening tool.

    2013, 40 (6):  492-495.
    Abstract ( 1320 )   PDF (596KB) ( 4762 )
With the progress of assisted reproductive technology and the increase of women′s reproductive age,the incidence of multiple pregnancy is also increased. Multiple pregnancy increased the morbidity and mortality of maternal-fetal complications. Therefore, prediction of mater-fetal complications and the standardized management of multiple pregnancy in the early term helps to reduce the maternal-fetal risks including abortion, anemia, preeclampsia and gestational diabetes mellitus. According to recent studies, the management of multiple pregnancy in the early term should pay attention to chorionic judgment, screening of fetal complications(screening for aneuploidies and ultrasound scanning), prediction of mater complications and standardized monitoring during early pregnancy, thereby reducing the mater-fetal complications and improve mother and child outcome.

Recent Advances in Retinol Binding Protein-4 and Gestational Diabetes Mellitus
    ZHOU Ping;CHEN Dan-qing
    2013, 40 (6):  496-499.
    Abstract ( 1345 )   PDF (489KB) ( 4733 )
Retinol binding protein-4 (RBP4) is a newly discovered adipocytokine mainly secreted by liver and fat cells, which can regulate glucose and lipid metabolism. RBP4 is closely related to insulin resistance and metabolic syndrome. Gestational diabetes mellitus (GDM) is a common pregnancy complications, its incidence is increasing, but its pathogenesis is unclear, which is considered to be associated with insulin resistance, regardless of compensatory insulin secretion. Recent studies on adipokines including RBP4 have suggested that RBP4 is not only the association factor in obesity and diabetes mellitus and insulin resistance, but also the novel factor closely related to the pathogenesis, prognosis and risk of type-2 diabetes mellitus and metabolic syndrome in the women with previous GDM and offspring of GDM. However,RBP4 is still a controversial adipokine.

    Abstract ( 1309 )   PDF (668KB) ( 4772 )
Objective:To examine maternal serum concentrations of leptin,beta-human chorionic gonadotropin(β-hCG) and ADAM12 between second and third trimester of pregnancy, and investigate whether an appropriate combination of those markers may be an effective means of screening for preeclampsia(PE). Methods:Samples from 189 pregnant women who presented to Fujian Maternity and Child Health Hospital for prenatal care from June 2007 to May 2008 were included in this prospective study,of which 25 were subsequently developed PE as case group and 164 were normal pregnant women as control group. The maternal serum was conducted between their second(between 20th to 24th week of pregnancy) and third(between 30th to 34th week of pregnancy) trimester of pregnancy. The cutoff point was determined using a receiver operating characteristic curve. Results:①The second trimester level of serum β-hCG and ADAM12 in case group were significantly higher than normal group(P<0.001). ②The third trimester level of serum leptin,β-hCG and ADAM12 in case group were significantly higher than normal group (P<0.001). ③ The third trimester level of serum leptin,β-hCG and ADAM12 in case group were significantly higher than their second trimester level(P<0.05). The third trimester level of serum leptin in normal group was significantly higher than their second trimester level (P<0.001). ④The cutoff concentration of β-hCG was 32 μg/L and ADAM12 was 818 μg/L in second trimester of pregnancies. Combining the values of β-hCG and ADAM12 to form a screening test would detect an estimated 82.61% of affected pregnancies. There was no statistically significant difference between combining β-hCG and ADAM12 AUC and single β-hCG or ADAM12 AUC(P>0.05).⑤The cutoff concentration of leptin was 23 μg/L, β-hCG was 37 μg/L and ADAM12 was 900 μg/L in the third trimester of pregnancies. Combining the values of leptin,β-hCG and ADAM12 to form a screening test would detect an estimated 92.31% of affected pregnancies. AUC of combined leptin,β-hCG and ADAM12 is greater than the single leptin or β-hCG leptin(P<0.05), but there was no statistically significant difference between combined leptin,β-hCG and ADAM12 and single ADAM12 (P<0.05).⑥Combining the second and third trimester levels of serum β-hCG to form a screening test would detect an estimated 81.25% of affected pregnancies. Combining the second and third trimester levels of maternal serum ADAM12 to form a screening test would detect an estimated 90.48% of affected pregnancies. Conclusions:This study shows that measuring levels of β-hCG and ADAM12 during the second and third trimester of pregnancy are useful in clinical practice to identify pregnant women who will develop preeclampsia.

The Role of Transmembrane 4 Superfamily Members in the Embryonic Implantation
    LIANG Ying-qiu;GAO Tian-yang
    2013, 40 (6):  521-524.
    Abstract ( 1212 )   PDF (602KB) ( 4748 )
The implantation of the blastocyst into the maternal uterus is a crucial step in mammalian reproduction. The successful establishment of pregnancy requires an interaction between the blastocyst and receptive uterus, then trophoblasts invade the endometrium accurately. And the decidualization of endometrium is considered a major factor in this process. Similar to tumor metastasis, embryonic implantation also include identification, adhesion, invasion and transfer process. Differently, the invasion of trophoblast has a “self-limited” cell behavior, and it is exactly controled by a complex network of various cells and molecules. Transmembrane 4 superfamily members play an important role in a series of biological actions, such as cell adhesion, invasion and transfer, not only in the tumor metastasis, but also in the embryonic implantation.

The Role of CD4+T Cells in Unexplained Recurrent Spontaneous Abortion:an Update
    WANG Wen-juan;LIN Qi-de
    2013, 40 (6):  525-529.
    Abstract ( 1407 )   PDF (695KB) ( 4707 )
The human gestation is a semi-allograft and hence antigenically foreign to the mother. Therefore, the process of implantation may include mechanisms to prevent allograft rejection, but once the immunological tolerance becomes imbalanced, pathological pregnancy, such as spontaneous abortion could occur. Unexplained recurrent spontaneous abortion (URSA) is thought to be caused by immunological rejection of the fetus by the mother. CD4+ T cells which include Th1, Th2, regulatory T cells (Treg) and Th17 cells, play important roles in allograft rejection. Maternal peripheral blood and decidual CD4+ T cells augmentation have been implicated in URSA. Mechanisms of immune tolerance is in the deepening, from the Th1/Th2 immune balance to the negative regulation of Treg cells. Recently, the Th17/Treg balance, has been described to paly an important role in the immunological rejection. In this paper the role of CD4+ T cells in URSA is reviewed.

Maternal Decidual Microenvironment and Trophoblast Cell Invasion
    ZHU Xiao-ming;HAN Tao
    2013, 40 (6):  530-533.
    Abstract ( 1353 )   PDF (580KB) ( 4718 )
During normal pregnancy, placental trophoblast cells have the ability to invade-like tumor cells. However, unlike tumor invasion, trophoblast cell invasion to the maternal deciduas is under highly control and is a normal physiological behavior. The overinvasion and inadequate invasion can lead to such pathologies of pregnancy as choriocarcinoma, miscarriage and preeclampsia, respectively. Nowadays, the mechanisms responsible for trophoblast cell invasion to maternal deciduas remain unidentified. Evidence indicates that the maternal decidual microenvironment exerts important influence over trophoblast cell invasion. The further study will be helpful to understand the pathogenesis of pathological pregnancy.

    Abstract ( 1243 )   PDF (652KB) ( 4673 )
Birth defect is one of the main causes of the death of perinatal infant, and it is also supposed to be the major cause that leads to stillbirth, deadbirth and affect the growth of neonatals. It relates to environment factors, genetic factors,or the interaction of each other. For recent years, more and more researches have focused on the environment risk factors leading to birth defects. It was found that, birth defects were related to harmful physical or chemical factors(such as radiation, hyperthermia, lead, cadmium, zinc and folic acid),maternal active and passive smoking, exposure to organic solvents and environmental pollution or exposure to agricultural toxic, drinking water disinfection by-products and so on. This paper aims to investigate the relationship between environment and birth defects,to provide a theoretical basis for reducing birth defects.

    2013, 40 (6):  541-544.
    Abstract ( 1100 )   PDF (792KB) ( 4708 )
Ectopic pregnancy is a common gynecologic acute abdominal disease, and along with the increase of gynecological inflammation, sexual transmitted disease, uterine cavity operation and caesarean section, the incidence of special ectopic pregnancy(SEP) is increasing over past several years. SEP includes caesarean scar pregnancy, ovarian pregnancy, rudimentary uterine horn pregnancy, cervical pregnancy and abdominal pregnancy. Some ectopic pregnancies are especially difficult for early diagnosis and have the risk of mis diagnosis, thus becoming life-threatening,because they have generally few apparent clinical features and signs which,if any,are not typical. With the application of ultrasonic skills within the diagnosis of pregnancy and the widely acceptance of the awareness of bearing and rearing better children, the threats of ectopic pregnancy is reducing year by year. The three major clinical features are menopause, abdominal pain, and vaginal bleeding, and different sort of SEP has its typical features each. It is an urgent issue for gynecological surgeons and emergency department of general hospital that the typical features of SEP,early diagnosis and timely treatment be emphasized.

Research Progress of the Progesterone Resistance Mechanisms of Endometriosis
    HI Xiu;XU Wei;DAI Hui-hua;WANG Xiu-li
    2013, 40 (6):  548-550.
    Abstract ( 1308 )   PDF (599KB) ( 4762 )
Endometriosis(EMs) is defined by the presence of visible endometrial implants outside the uterus, mostly occurred in the pelvic organs. The cyclical bleeding of ectopic endometrial lesions leads to serious pelvic pain and infertility of women. EMs is widely viewed as an estrogen-dependent disorder. Progesterone is the classic drug to antagonist estrogen. But in the clinical practice, the majority of the endometriosis patients show low reaction or non-reaction on progestin therapy. Attenuation of progesterone response at the level of the endometrium may be expected to have a deleterious impact on endometrial receptivity and fertility. These phenomena are called progesterone resistance of EMs. The progesterone resistance mechanism involves the defect of the progesterone receptor(PR),the change of progesterone receptor co-regulators and the genetic and environmental factors as well as ERα/ERβ. The aim of this review is to highlight recent findings regarding the progesterone resistance mechanisms and to provide theoretical basis for overcoming progesterone resistance.

    2013, 40 (6):  551-554.
    Abstract ( 1200 )   PDF (604KB) ( 4720 )
Ovarian cancer is the leading cause of mortality in the gynecologic malignant tumor. 80% of patients are already in the middle or late stage of ovarian cancer when presenting with symptoms. Qualitative and location diagnosis in the early stages of the cancer is one of the key factors to improve the survival rate of patients. Nano-scale contrast agent has the characteristic of the strong blood vessels penetrating ability and confocal imaging. Tumor-specific targeting microbubbles could be generated by conjugating tumor-specific antibodies or ligands into the surface of the microbubbles which can results in selectively accumulation, long resident time in cancer tissue, so it can generate a significantly enhanced signal in the target area. Tumor-specific targeting microbubbles entrapped therapeutic genes or drug could be destroyed by ultrasonic wave and released genes or drug in the target site, which results in improving treatment effect.

Methods:Hela cells were cultured in vitro, added the different concentration of puromycin to screen,,and to determine the best screening concentration;Hela cells were tested from 24-well plates, threw away the old culture medium, and added 400 μL of new medium to the plates, and then added four kinds of lentivirus solution(ALDH-1, ALDH1-1719, ALDH1-740, ALDH1-921,and there was a kind of lentivirus which has the best interfering effect) and one kind of negative control lentivirus solution (LV3-NC) to the plates, at last added Polybrene liquid to them. Every lentivirus solution volume was added about 50 μL (each of viral titers were 1×108 TU/mL),and the Polybrene was added about 0.5 μL (concentration of 5 mg/L) respectively. Fresh culture medium was changed after 24 hours, and observe the fluorescence after 72 hours. At last, using the best screening concentration of puromycin to screen the transfection Hela cells, and to get the stable transfection cell lines. Results:The best screening concentration of puromycin was 0.9 mg/L,lentiviral vectors were successfully transfected into Hela cells, and the stable transfected cell lines were successfully obtained after two weeks. Conclusions:Hela cells which has stable interference of ALDH-1 were successfully constructed.

    Abstract ( 1388 )   PDF (446KB) ( 4677 )
Objective:To investigate the perception, attitude, and utilization of oral contraceptives(OC) among population with history of breast cancer and ovarian cancer. Methods:All of 92 females who had history of breast cancer and ovarian cancer among first level relatives were surveyed on perception, attitude, behavior, and information need. Results:There were 65.2% of participants used OC,and 40.6% of them reported that history of cancer was a major reason for not using OC. Most of participants wanted to know more information about OC and preferred to get information through health education handbook. Conclusions:Among females with history of breast cancer and ovarian cancer,it was helpful to choose ideal contraceptive methods when they had a good perception on OC.

    Abstract ( 1231 )   PDF (423KB) ( 4656 )
Objective:To investigate the influence of misoprostol combined with oxytocin on serum levels of angiotensin Ⅱ (AngⅡ) and coagulation factors after parturient postpartum, which is a useful reference for effective treatment of postpartum hemorrhage. Methods:120 parturients with postpartum hemorrhage from our hospital during Dec 2010 to Jan 2013 were included in this study. Those high-risk factors were pregnancy hypertension, placental abruption, placenta previa, fetal macrosomia and multiple pregnancy. Those cases were randomly divided into three groups, each with 40 cases. Parturients in group A were immediately administrated with misoprostol 400 μg through rectum after baby delivery. Parturients in group B were immediately administrated with oxytocin 20 U through palace intramuscular,at the same time, with oxytocin 20 U through intravenous infusion. Parturients in group C were immediately administrated with oxytocin 20 U through palace intramuscular and misoprostol 400 μg through rectum. The postoperative bleeding volume was measured by volum and weight after postpartum 2 h and 24 h. The serum levels of angiotensin and coagulation factors were measured by ELISA. The systolic pressures before childbirth and post partum were observed. Results:There were not significant differences in the serum levels of AngⅡbetween antepartum and postpartum, in both group A and group B(P>0.05) while this difference was significant in group C(t=7.196,P=0.009). The same status was found in the serum levels of two coagulation factors in three groups. The hemorrhage volumes post partum 2h, 24h of group A and group B were significantly increased than that of group C (P<0.05). The differences of those systolic pressures before childbirth and post partum 2h, in three groups, were not significant (P>0.05). Conclusions:Misoprostol combined with oxytocin can significantly increased serum levels of angiotensin and coagulation factors when to treat effectively postpartum hemorrhage.
