    Female Sexual Dysfunction:Classification and Definitions

    Female sexual dysfunction (FSD) is particularly prevalent among women, adversely affecting on female physical and mental health. The international diagnostic categories of FSD include The World Health Organization International Classifications of Diseases-10 (ICD-10),The Diagnostic and Statistical Manual of Mental Disorders Ⅳ-Ⅴ(DSMⅣ-Ⅴ) and The Consensus-based Classification of Female Sexual Dysfunction(CCFSD). Based on the four-phase class of the normal female sexual response,these classification systems have classified female sexual dysfunctions into the following major categories:sexual desire disorders,sexual arousal disorders,orgasmic disorders and sexual pain disorders. The diagnostic and classification systems have been developing with our unceasingly expanding knowledge of female sexual dysfunction. However,these diagnostic and classification systems still have some defects in some degree,thus,further revision and new focus need to be launched.

    Pay Attention to Female Dyspareunia

    Dyspareunia is a common symptom in clinical obstetrics and gynecology,it′s a constituent part of female sexual dysfunction. The definition is the related pain to the penis into vagina or trying to enter. However,currently there is no general,unified and comprehensive evaluation guidelines. Dyspareunia has special requirements for the history collection,physical examination and laboratory tests. Chinese gynecologists should keep up with the current international dynamic and make accurate diagnosis and treatment,it is important to not only deal with organic disorders by surgery,but also pay attention to the psychological treatment.

    The Diagnosis of Female Sexual Dysfunction

    The fourth edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-Ⅳ) classified female sexual dysfunction (FSD) into the following categories:Sexual Desire Disorders,Sexual Arousal Disorders,Orgasmic Disorders and Sexual Pain Disorders. The DSM-Ⅴ was officially published in May 2013. The proposed changes include:deletion of sexual aversion disorder and classified it as an anxiety disorder;redefining female orgasmic disorder;merging desire and arousal disorders into one definition as sexual interest arousal disorder;combine vaginismus and dyspareunia into genito-pelvic pain disorder. Gynecological doctors should be able to make FSD diagnosis,pay more attention to the discovery of organic disease and the desensitization treatment of vaginismus,and provide referral to a clinical psychologist for the patients with sexual desire disorder,sexual arousal disorder or orgasmic disorder.

    Focus on the Impact of Gynecological Surgery on Female Sexual Function

    Sexual disability in the female patient is a multifactorial problem. Benign gynecological diseases such as uterinemyoma,endometriosis,urinary incontinence,pelvic organ prolapse,and gynecological malignant tumor have a negative impact on various parameters of sexual health. Gynecologist should perform such kind of surgery procedure to address the problem,including hysterectomy,bilateral salpingo-oophorectomy,pelvic organ reconstruction and tumor radical-surgery. Although it could relieve the symptoms related to the disease, but the patient sexuality function may be significantly negatively influenced occasionally. As for cancer patient treatments,such as excision surgery of reproductive organs and sex gland,surgery combining with chemotherapy and(or) radiation therapy appear to have serious damage on the sexual morbidity. Female sexual health is a topic worthy of high attention. It is critical that gynecologist and psychologist should reinforce the psychological counseling before and after surgery. Treatments should be taken for postoperative sexual dysfunction.

    The Influence of Infectious Diseases on Female Sexual Function

    Female sexual dysfunction could be caused by a variety of reasons. Due to clinical symptoms,signs and psychological barriers,female infectious diseases,as common female diseases,have become one of the important factors in sexual function,which have physical and psychological impact on patients and have serious influence on women′s quality of life. Common female infectious diseases,such as vulvitis,vaginitis,cervicitis,pelvic inflammatory diseases and sexually transmitted diseases,could impact on different phases of sexual response cycle,leading to sexual difficulties and even female sexual dysfunction. As for treatment to female sexual dysfunction caused by infectious diseases,exact anti-infective treatment should be needed, without ignorance of patient′s psychological counseling related to sex life.

    Sex is a normal action related to natural instincts,physiological and mental phenomena in human. It is even an eternal pursuit for man and woman to enjoy sex itself and the happiness associated with sexual life in their long life after they bear children. In general,good and healthy male sexual ability is the first factor for both man and woman to enjoy sex. The physiological and psychological differences in sexual reaction between man and woman can be minimized by their joint effort. Male sexual dysfunction,such as erectile dysfunction and ejaculatory dysfunction,unavoidably result in disappointment for both man and woman. Man and woman should be coordinated by the common sexual values and common beliefs. The good treatment for sexual dysfunction should be comprehensive with the help of modern medicines,and joint effort and continuous practice by both man and woman.

    Roles of Tight Junction in Normal and Pathological Pregnancy

    Tight junction is a fundamental junctional complex of the adjacent epithelial cells consist of several protein families such as transmembrane proteins occludin,claudin and peripheral proteins ZOs. Tight junction performs specialized functions including maintenance of paracellular barrier functions and cell polarity principally. The structure and function of tight junction can bechanged by some physio-/pathological factors. Tight junction has already been detected in human uterine endometrium,placenta and luteal cells. Abnormality of tight junction may correlate with endometrial receptivity,placental implantation and luteal function and also has been involved in the pregnancy complications.

    Fluorescence in situ hybridization is a new non-radioactivity molecular cytogenetic technique based on radioactivity in situ hybridization since 1980. It′s a production combined with molecular biology,cytogenetics and immunology. This technique is applied broadly in molecular biology,medicine,cytogenetics and so on. It has been applied in the study of preimplantation genetic diagnosis,prenatal diagnosis,spontaneous abortion and congenital heart diseases. This review give a brief overview of its application in obstetric field.

    Developmental Origin of Polycystic Ovary Syndrome

    Polycystic ovary syndrome(PCOS) is a common reproductive endocrine and metabolic disorder. On the basis of experimental observations in the prenatally androgenized sheep and rhesus monkey,and supported by data from human studies. We therefore conclude that although PCOS is a complex,heterogeneous disorder,most if not all of the clinical and biochemical features can be explained on the basis of a developmental disorder of ovarian androgen production. This fetal (and/or prepubertal) androgen excess “programmes” the hypothalamic-pituitary control of LH,enhances visceral fat distribution (thus predisposing to insulin resistance and anovulation) and causes the clinical manifestations of hyperandrogenism in adulthood. Other secondary genetic and environmental (particularly dietary) factors may interact with this underlying linear process to modify the final phenotype and produce the heterogeneous nature of the syndrome.

    Lactobacillus rhamnosus GR-1 and Lactobacillus reuteri RC-14 for Treatment and Prevention of Vaginal Infections

    The vaginal microbiota consist of a large number of lactic acid bacteria,primarily species of Lactobacillus. A key ecological function is the maintenance of a low pH in the vagina,which is important for resistance against pathogenic organisms. Vaginal infections commonly occur in women of reproductive age and bacterial vaginosis (BV) or yeast vaginitis is the most common cause for women to seek medical care. This review focuses on the usage of oral probiotics Lactobacillus rhamnosus GR-1 and Lactobacillus reuteri RC-14 for improvement of vaginal health,as adjunctive treatment in symptomatic disease and as stand-alone treatment for restoring and maintaining a healthy vaginal microbiota. In vitro experiments demonstrate that strains GR-1 and RC-14 are able to adhere to epithelial cells,and to inhibit pathogens-particularly uropathogens-by producing bacteriocin-like substance and displacing biofilms. Data from use in humans reveals that both strains are able to survive gastrointestinal passage and translocate to the vagina(from the gut via the perineum)as viable bacteria. This has been demonstrated by isolating GR-1 and RC-14 from fecal and vaginal samples following oral administration.

    Angiogenic factors and anti-angiogenic factors have been studied in many areas. Now,they have become a hot spot in the Pathological obstetric,many research are on the preeclampsia in maternal plasma,but the role of stillbirth in them is still ralely repored. This paper mainly discusses the clinical etiology of stillbirth,the significance of PIGF、 sVEGFR-1、sEng change and the reasons for stillbirth of amniotic fluid and maternal serum of endothelial growth factor receptor-1、placental growth factor and soluble endoglin sEng concentration. We make a detailed study of the relationship between changes of anti-angiogenic and Anti-angiogenic factor and the occurrence of stillbirth,open up new ideas for the etiology and pathogenesis of stillbirth.

    With the development of molecular techniques, the research about the role of microRNA (miRNA) in tumor occurrence and development have became the new hot spot in bioscience field. MiR-183 gene cluster (miR-183 family) is a highly conservative gene cluster,which includes miR-183,miR-182 and miR-96,that located in 7 chromosome. Researches have shown that abnormal expression of miR-183 gene cluster can be found in variety of cancinomas. Based on the newest report at domestic and oversea, we review some recent literatures about the function of miR-183 family and the correlation between the miR-183 family and cancer.

    Evaluation of Clinical Value of Human Epididymis Protein 4 in Patients with Endometrial Cancer

    Endometrial cancer is one of the most common gynecologic malignancies,and it remains one of the most important causes of mortality in women worldwide. In recent years,the incidence of endometrial cancer has been increased and the age of onset of endometrial cancer has decreased. So it is essential for early diagnosis and treatment of endometrial cancer,but until now,there is still lack of sensitive and specific serological markers. Recent studies have demonstrated that human epididymis protein 4(HE4) was highly expressed in endometrial cancer,and lowly expressed in normal endometrium and benign endometrial lesions. Studies focus on whether HE4 can be used as endometrial cancer early diagnosis and prognosis marker now. This review will briefly summarize the clinical value of HE4 in diagnosis and prognosis of endometrial cancer.

    Methylation Alterations of Human Papillomavirus in Cervical Carcinogenesis

    The pathogeny of cervical cancer is persistent high-risk HPV infection. However,most of HPV infection can be cleared by the host′s immune system,only minority will intergrate into the host′s DNA and transform the epithelial host cells to malignant cells. Together with further genetic and epigenetic alterations of HPV play a role in virus DNA intergration and carcinogenisis. This paper reviewed how HPV epigenetic alterations can affect the expression of papillomaviral as well as the relation to the multistep process of carcinogenesis. HPV DNA methylation level affected the carcinogenisis and progression of cervical cancer. Biomarker roles in clinical diagnosis and prognosis might be assigned to the epigenetic pattern of HPV genes.

    Uterine myoma is a kind of disease that is common and frequently happened in the reproductive-age women, and its symptoms diversity greatly affects the quality of women′s life. That uterine myomas has a bad impact on the reproductive ability and has always been a hotspot issue to discuss. Laparoscopic surgery has gained a rapid development in recent years. Because there is rich neurovascular bundle under psedocapsule, intracapsular laparoscopic myomectomy can benefit myometrial muscle for its small wound and less bleeding merit, which makes myometrial muscle to recover faster. Besides, this method can especially meet the needs of patients who suffered from uterine myomas and also want to keep the ability to be pregnant, so it should be used in a more wide application. Now the domestic and foreign research advances of this technology are reviewed in this article.

    Meta-analysis for the Effect and Side Effect of Gestrinone

    Objective:To evaluate the value of gestrinone in the treatment of endometriosis (EMs). Methods:We searched OVID, MEDLINE and CNKI for RCTs. This meta-analysis is performed based on “Cochrane Handbook for Systematic Reviews of Interventions”. Results:Gestrinone has better curative effect compared with marvelon (RR=8.270,95%CI:1.046-65.384),but its efficiency is inferior to GnRHa (RR=0.888,95%CI:0.845-0.933),danazol (RR=1.043,95%CI:0.949-1.146),mifepristone (RR=0.972,95%CI:0.943-1.003) and traditional Chinese medicine (RR=0.954,95%CI:0.905-1.006) show no difference from gestrinone in efficiency. Gestrinone and mifepristone (RR=0.913,95%CI:0.730-1.140) have equal pregnancy rate,but more women get pregnant after treated by GnRHa when compared with that by gestrinone (RR=0.792,95%CI:0.657-0.954). Gestrinone shows a higher incidence of side effect (liver dysfunction) than mifepristone (RR=1.865,95%CI:1.453-2.394) and marvelon (RR=8.270,95%CI:1.046-65.384),but there is no difference when compared with GnRHa (RR=1.308,95%CI:0.965-1.772). The AFS scores of two dosage regimens of gestrinone are the same (SMD=0.185,95%CI:-0.491-0.861). Conclusions:As a second-line drug for treating EMs,the effeciency of gestrinone is inferior to GnRHa. Increasing the dosage shows no improvement on efficiency. It′s still an option when the side effects are tolerable.

    The Expression of Neuronal Protein Degradation Product 9.5 in Eutopic Endometrium and Ectopic Endometrium of Endometriosis

    Objective:To investigate the expression of protein gene product 9.5 (PGP 9.5) in situ function layer and ectopic endometrium of endometriosis,and observe the relationship of the expression of PGP 9.5 and the pelvic pain. Methods:44 endometriosis patients enrolled in this study (pain group 28 cases,without pain group 16 cases),to detect the expression of PGP 9.5 in situ function layer and ectopic endometrium of patients were determined by immunohistochemical methods of Envision TM,20 patients with uterine fibroid without pelvic pain were included as the control group. Investigate the relationship of PGP 9.5 in endometriosis and the PGP 9.5 and the pelvic pain. Results:The positive rate of PGP 9.5 expression in endometriosis group situ function layer was 72.9%,and the positive rate of PGP 9.5 expression in control group situ function layer was 25.0%,which has significant difference (P<0.05). The positive rate of PGP 9.5 expression in EMs group ectopic endometrium was 81.8%,and the positive rate of PGP 9.5 expression in endometriosis pain group ectopic endometrium was 92.9%,compared with the without pain group 62.5%. Which has significant different (P<0.05). EMs in ectopic group and ectopic endometrial nerve fiber density were higher than control group (all P<0.01). The expression of PGP 9.5 has nothing to do with the staging and location of the EMs (all P>0.05). EMs patients′ pain has nothing to do with the lesion location (P>0.05),and has nothing to do with the density of nerve fibers (P>0.05). Conclusions:The expression of PGP 9.5 had a significant increase in the endometriosis situ function layer and ectopic endometrium. At the same time,its increased expression had a close relationship with the pelvic pain.

    The Expression and Clinical Significance of ESM-1 in Cervical Cancer

    Objective:To explore the expression and clinical significance of endothelial cell specific molecule-1 (ESM-1) in human cervical cancer. Methods:200 patients were randomly selected in Xiaogan Central Hospital,normal women,chronic cervicitis,CIN and cervical cancer were 50 cases separately,using ELISA method and immunohistochemical method to detect the expression of ESM-1 protein,and comparing its expression between different age,pathological grades,clinical stages and lymph node metastasis. Results:Compared with the normal cervical tissue,the cervicitis tissue and the CIN tissue,the expression of ESM-1 in cervical cancer was significantly increased(P<0.05). And its expression was statistically correlated with age,pathological grades,clinical stages and the lymph node metastasis (P<0.05). Conclusions:ESM-1 is expected to become one of the important indicators in the development of cervical cancer.

    Research of Tumor Markers for Endometrial Cancer Risk Evaluation

    Objective:To explore serum human epididymis protein 4 (HE4),cancer antigen 125 (CA125) for uterine endometrial cancer risk assessment. Methods:With electroluminescent assay in 453 patients with myoma of uterus,126 cases of adenomyosis,26 cases of endometrial hyperplasia patients,10 cases of endometrial polyps patients,212 cases of endometrial cancer patients and 72 cases of healthy women of serum HE4 and CA125 levels. Results:The serum level of HE4 expression in endometrial carcinoma were higher than that of the other groups,the difference was statistically significant (P<0.01). The serum level of CA125 expression in adenomyosis were higher than that of the other groups,the difference was statistically significant (P=0.000). Endometrial carcinoma patients serum HE4 expression level and FIGO stag were a significant positive correlation (rs=0.538,P=0.000). HE4 and CA125 combined detection of endometrial cancer sensitivity were higher than single HE4 detection sensitivity. Conclusions:Single serum HE4 and CA125 detection has a certain significance to the early diagnosis,staging and prognosis of endometrial carcinoma,which can be used as an important index for the diagnosis of endometrial cancer.

    Expression and Clinical Significance of MicroRNA-210 in Ovarian Cancer

    Objective:Detection the expression of miR-210 and investigation its roles in epithelial ovarian cancer(EOC). Methods:Poly(A)-RT-qPCR was used to detect the expression of miR-210 in epithelial ovarian cancers and benign epithelial ovarian cysts and normal ovarian tissues. Results:miR-210 in ovarian cancer group was lower than that of benign ovarian tumors and normal ovarian group,significant difference among the three groups(F=893.213,P=0.000). With the progress of the clinical staging of ovarian cancer tissues and tissue differentiation,miR-210 expression there has a downward trend. Conclusions:Our data suggest that miR-210 expressed lowerly in EOC,it might play an important role as an anti-oncogene in the genesis and development of epithelial ovarian cancer. It is bound with lymphatic metastasis,stages of ovarian cancer and tissus grading.

    Clinical Observation of Dose-dense Chemotherapy in the Treatment of Advanced Epithelial Ovarian Cancer

    Objective:To investigate the clinical efficacy of dose-dense chemotherapy and standard chemotherapy regimen in the treatment of advanced epithelial ovarian cancer. Methods:Select 89 patients who were diagnosed by pathology with advanced epithelial ovarian cancer in The Fifth Central Hospital of Tianjin. The patients were randomly divided into dose dense group of 45 cases and a standard regimen in 44 cases,the age,pathological type and clinical stage of the two groups patients were not statistically significant (P>0.05). Standard solutions group:Paclitaxel 180 mg/m2(intravenous drip for 3 h),then the use of carboplatin AUC 6(intravenous drip for 1 h,the first day),the interval of 3 weeks,a total of 6 courses. Dose dense group:Paclitaxel 80 mg/m2 1,8,15 days + carboplatin AUC 6(intravenous drip for 1 h,the first day),during the interval of 3 weeks,a total of 6 courses. The two groups were treated and followed up for 3 years,compared the two groups of patients with long-term efficacy,therapeutic effect and adverse reaction. Results:The total remission rate of dose dense group was higher than that of the standard regimen group,the difference was statistically significant(P<0.05). Comparison of progression free survival and 3 year survival rate,the dose dense group was significantly higher than that of standard solutions group (P<0.01). Although the incidence of the adverse reaction rate was also higher than that of the standard regimen group,there was statistically significant but only Ⅲ~Ⅳ anemia incidence rate difference between the two groups (P<0.05),the rest incidences were no significant difference(P>0.05). Conclusions:The dose dense chemotherapy can effectively improve the 3 year survival rate of advanced epithelial ovarian cancer patients,delaying tumor recurrence.

    The Clinical Diagnostic Evaluation of Hysterosalpingography and Hysteroscopic Hydrotubation in Fallopian Tube Infertility

    Objective:To compare the clinical value of hysteroscopic hydrotubation (HH) and hysterosalpingography (HSG) in tubal infertility diagnosis. Methods:1 452 infertility cases,which was diagnosed from Nov. 2009 to Nov. 2011 in the Affiliated Hospital of the Chinese People′s Armed Forces Logistic College were selected randomly according to the inclusion criteria,and every patient under intravenous anesthesia was treated with HH under the monitor of B ultrasound. Patients suffered from tubal infertility underwent hysteroscopic and laparoscopic operation,and patients whose oviducts were patent were arranged to conceive,and would be examined by hysteroscope and laparoscope one year later if they cannot conceive naturally. The diagnosis rate of HH and HSG will be compared with reference to the result of hysteroscope and laparoscopes operation. Results:The coincidence rate, sensitivity and specificity of HH is 86.1%(2 500/2 904),98.7%(2 370/2 401),and 58.6%(295/503) respectively,and 74.3%(2 130/2 866),93.7%(2 252/2 404),42.4%(196/462) is that of HSG. Statistical difference is found between HSG and HH (P<0.01). Conclusions:HH is more accurate when diagnosing tubal patency than HSG,and HH does no obvious harm to patients,and it can observe the abnormality in uterine cavity and treat directly,so HH is superior to HSG on initially diagnosis of tubal infertility.

    Effects of Laparoscopy Guided Hysteroscopic Tubal Catheterization

    Objective:To evaluate the rate of postoperative recanalization in different parts by laparoscopy guided hysteroscopic fallopian tube catheterization. Methods:From January 2008 to December 2009,880 infertile patients with 1 760 fallopian tubes were performed by laparoscopy guided hysteroscopic tubal catheterization. Postoperative recanalization rate was observed. Results:The total rate of selective tube recanalization was 74.5%. The recanalization rate in tubal fimbria,interstitial and ampulla was 86.0%,65.8% and 36.7% respectively. The postoperative spontaneous pregnancy rate was 39.6% in 2-year follow up. Conclusions:The highest rate of tubal recanalization by laparoscopy guided hysteroscopic tubal catheterization was fimbria block,and then interstitial and ampulla part.
