摘要： 子宫内膜异位症、异位囊肿、子宫肌瘤、子宫内膜息肉、子宫纵隔和宫腔粘连与女性不孕症有关，手术治疗与药物治疗相比可以提高术后妊娠率，降低复发率。生殖手术在提高体外受精（IVF）治疗结局和改善生育力方面有重要作用。卵巢内膜异位囊肿的存在不会影响IVF的妊娠率，但手术治疗对提高生育能力和减轻疼痛是非常有效的，术中要注意预防粘连的形成和卵巢损伤。子宫内膜和宫腔病变会影响IVF结果，IVF失败一次时应该行宫腔镜检查，通过宫腔镜手术有效纠正宫腔病变。育龄女性施行任何妇科手术时都应该遵循微创和保护生育能力的原则。

Abstract: Endometriosis，endometriotic cyst，uterine fibroids， endometrial polyps，septum and intrauterine adhesions were associated with female infertility. Surgery treatment could improve postoperative pregnancy rate and decrease recurrence rate compared with drug therapy. Reproductive surgery played an important part in improving IVF outcomes and fertile ability. Ovary endometriotic cyst would not affect the pregnancy rate of IVF， but it′s efficient for improving reproductive ability and decreasing pain by surgery. Adhesions and ovary damage should be prevented in the surgery. Endometrium and intrauterine diseases could affect IVF outcomes. Hysteroscopy examination should be used when IVF failed more than once to correct intrauterine diseases. Gynecological operation provided to women in reproductive age should follow the fundemental of microdamage and reproductive ability protection.
