    Uterine Smooth Muscle Tumor of Uncertain Malignant Potential: Status and Controversy

    Uterine smooth muscle tumor of uncertain malignant potential (STUMP) is the neoplasm with pathological features that preclude an equivocal diagnosis of leiomyosarcoma, but that do not fulfill the criteria for leiomyoma or its variants, and raise concerns that the tumor may behave in a malignant fashion. The incidence of STUMP is relatively low. The clinical features and imaging manifestations of STUMP are lack of specificity. In most cases, STUMP grow slowly, and the prognosis is worth expecting. However, the actual course of this disease is difficult to predict, and there is a risk of recurrence and malignant transformation. The diagnosis of STUMP depends solely on histopathological examination, but no consensus has been reached on pathological diagnostic criteria. Immunohistochemical molecular typing and genomic analysis may be helpful for the assessment of prognosis factors, but the evidence is limited and further researches are needed. Although there are no guidelines or consensus governing the treatment of STUMP, total hysterectomy with or without bilateral salpingo-oophorectomy is the standard treatment if fertility is completed, whereas myomectomy alone can be taken into consideration in young patients who desire to preserve childbearing potential. Regular follow-up after surgery is important to detect missed diagnosis and tumor recurrence in time.

    Roles of EZH2 Histone Methyltransferase in Tumorigenesis

    The Polycomb group(PcG)protein family is a group of gene regulatory elements playing roles in embryonic development,which functioning in tow distinct protein complexes termed Polycomb-repressive complex 1(PRC1) and Polycomb-repressive complex 2(PRC2). EZH2 is the catalytic subunit of PRC2,which is a highly conserved histone methyltransferase that targets lysine-27 of histone H3. Most studies on human tumors show that EZH2 is over-expression in a variety of tumors,including prostate and breast. Although the mechanism of EZH2 with cancer progression is not determined,functional link between EZH2-mediated histone methylation and DNA methylation indicate which involved in the gene silencing machinery implicated in tumor suppressor loss. The authors review the basic molecular biology of EZH2 and the findings of EZH2 in different cancers. And also discuss EZH2 connections to other epigenetic modifying enzymes,as well as the consequences of EZH2 overabundance and its potential roles in tumorigenesis.

    Endometrial cancer is one of the common malignant tumors of the female reproductive system. The most common pathological form of endometrial cancer is endometrial endometrioid adenocarcinoma. The prognosis of early endometrial cancer is better, but there are still a few patients with poor prognosis. Factors that affect the prognosis of endometrial cancer include histological grade, depth of myometrial invasion, lymphovascular space invasion (LVSI), cervical stroma involvement, and lymph node metastasis. Microcystic, elongated and fragmented (MELF) pattern is a special myometrial invasion pattern of endometrial endometrioid adenocarcinoma. Several studies have confirmed that the MELF pattern is associated with some adverse pathological factors that affect the prognosis, but the prognostic significance of MELF pattern remains unclear. Clinicopathological features and prognostic significance of MELF pattern are now discussed in this review in order to provide a theoretical reference for the related research of MELF pattern in the future.

    Large cell neuroendocrine carcinoma (LCNEC) of the cervix is a rare cervical malignancy. The prognosis of LCNEC of the cervix is very poor showing a high degree of malignity, a rapid progression, a high relapse rate and early blood spread. Its development is closely related to high-risk human papilloma virus infection. Although the cytologic abnormality can aid to the diagnosis of LCNEC of the cervix, the pathologic examination combined with immunohistochemical study is the main method for its diagnosis and differential diagnosis. There is no unified therapy at present, but a majority of scholars presume that operation combined with radiotherapy and chemotherapy can improve the survival rate.

    The incidence of cervical cancer ranks fourth in the world for female malignancies, and it is the main cause of cancer deaths among women in developing countries. In some areas where the economy and culture are relatively backward, the awareness and implementation rate of "step screening for cervical cancer" are poor, so that more than 1/3 patients are at stage ⅠB3 (FIGO stage in 2018) and above of the cervical cancer when they found it, and lost the best time for operation. The concept of neoadjuvant chemotherapy (NACT) before operation, which is in order to shrink tumors and eliminate micrometastases, has been widely accepted in many tumor species. It also has been practiced a lot in the treatment of cervical cancer in China. However, the concrete proposal of NACT for cervical cancer has not been unified yet. Paclitaxel is a commonly used drug in the chemotherapy of cervical cancer, which is highly toxic and poorly tolerated by patients. The nanoparticle albumin bound paclitaxel (Nab-paclitaxel), a small molecule nanoparticle which uses human serum albumin as the solvent, is poorly toxicity and highly efficiency. As a new formulation of paclitaxel, it is widely used. Now, this article summarizes the principles, the efficacy, adverse reactions and long-term application of Nab-paclitaxel and traditional solvent-based paclitaxel in NACT for cervical cancer.

    Advantages and Complications of Transvaginal Mesh Surgery

    With the increasing aging of the society, pelvic organ prolapse (POP) has become a common disease of elderly women.Transvaginal mesh (TVM) surgery, as a kind of minimally invasive surgery through natural channel for the treatment of POP, with the advantages of anatomical reduction, functional recovery and long-term curative effect, is widely used in recent 20 years. However, it has been warned twice by the US Food and Drug Administration (FDA) for mesh-related complications. The International Urogynecological Association/International Continence Society (IUGA/ICS) classification of mesh-related complications includes:exposure and erosion, hematoma, bladder, urethral and bowel injury, infection, chronic pelvic pain, and de novo dyspareunia. Mesh exposure and erosion are most common complications. It is worth thinking about whether transvaginal mesh should be used or not in China.

    The Apoptotic Pathways and Their Mechanisms

    Cell apoptosis exists in the whole life of multicellular organisms,which is capable to maintain homeostasis of tissues and organs through removing unwanted or damaged cells. Eukaryotic cells mediate apoptosis through four main pathways:the death receptor-mediated extrinsic pathway,the intrinsic mitochondrial pathway,granzyme B-mediated pathway and endoplasmic reticulum stress-mediated pathway. There are four important protein molecules involved in apoptotic pathways:caspases (the proteases which execute cell death),adapter proteins (caspase activators),Bcl-2 family proteins and Inhibitor of apoptosis proteins(IAPs). Cell apoptosis plays an important role in establishment of ovarian reserve and abnormal apoptosis inducing some diseases,such as endometriosis,ovarian cancer,et al. In this review we will discuss the apoptotic pathways and their mechanisms mentioned above.

    Diagnosis and Treatment of 6 Cases with Mirror Syndrome

    Objective:To explore the clinical features,diagnosis and treatment of mirror syndrome. Methods:The clinical data including clinical symptoms and physical signs,laboratory and ultrasound examination,gross and microscopic placental examination and outcomes of both mother and fetus in 6 cases with mirror syndrome at our hospital from 2009 to 2010 were analyzed retrospective,and relative literatures were reviewed. Results:6 cases with mirror syndrome was presented the triad of maternal edema,hydrops fetalis and hydropic placenta. 6 cases have several characteristics such as anemia and hemodilution[Hb(86.00±10.88) g/L和HCT 0.267 7±0.026 7], hypopoteinemia[ALB( 21.53 ± 3.12) g/L], albuminuria and proteinuria,elevated uric acid [UA(551.50±63.02) μmo/L].In addition,elevated blood pressure was present in 4 cases. Conclusions:Mirror syndrome can be diagnosed as the development of maternal edema in association with hydrops fetalis or hydropic placenta. Some cases can present preeclampsia-like symptoms.An important characteristic of mirror syndrome that distinguishes it from preeclampsia is maternal hemodilution. Once mirror syndrome is diagnosed,the cause of hydrops should be corrected as soon as possibly. Diuretic and limitation of fluid input volume also should be considered.

    Objective:To evaluate the effectiveness and feasibility on treatment of cervical incontinence by cervical cerclage surgery using cerclage tape. Methods:Case 1 owing to cervical incontinence was performed vaginal cerclage during 21w gestation,during 23w gestation she suffered from inevitable abortion and was performed vaginal cerclage again but failed. Finally she was performed abdominal cerclage at non pregnant condition. Case 2 had 5 times mid trimester abortion because of cervical incontinence. She was performed laparoscopic cerclage at non pregnant condition. Both surgeries were going smoothly.Results:Case 1 got pregnant 1 year and 3 months after surgery. At 35+6 w a live girl was born by cesarean section with indication of central placenta previa. Baby’s birth weight was 2 900 g.Case 2 was not pregnant for one year after surgery. HSG shown bilateral tube occluded. Liquid cannulation was performed twice afterwards but tube was still blocked. Conclusions:As for an alternative method, abdominal/laparoscopic cerclage at non pregnant condition is effective to treat cervical incontinence women who had history of mid-trimester abortion even failed of vaginal cerclage.

    Clinical Research on Transverse Annular Compression Sutures in the Lower Uterine Segment to Control Postpartum Hemorrhage at Cesarean Delivery for Complete Placenta Previa

    Objective:To evaluate the efficacy of transverse annular compression sutures(TACS)in the lower uterine segment in achieving homeostasis to control bleeding during cesarean delivery in women with complete placenta previa(CPP). Methods:Retrospectively analyze 60 cases who met the inclusion criteria with CPP during 2006,January to 2010,December. All the patients were divided into two groups:subsequent administration of either uterine packing(UP)or TACS. Blood loss,intervention time,and clinical efficacy were compared. Results:①There was no statistic difference between the UP group and the TACS group in gestational age at delivery,birth weight,the proportion of anterior placenta and the incidence of placenta accreta(P>0.05). ②No difference was found between the groups in oxytocin dose(P>0.05). However,carboprost usage amount was much more in the UP group(P<0.05). ③No difference was found between the groups in percentage of ante partum hemorrhage and the blood loss of antepartun. But there was greater blood loss in the UP group compared with the TACS group before the intervention,during the intervention,after the intervention,and throughout the cesarean delivery procedure(P<0.01). The percentage and quantity of blood transfused was also significantly higher in the UP group than in the TACS group(P<0.05 or P<0.01). ④The time taken to perform the uterine packing was significant longer than the time taken to perform TACS(P<0.05). The success rates were 91.7% and 97.2% for UP and TACS,respectively(P>0.05). ⑤Antibiotics using time and post-operative hospitalization time was significantly shorter in the TACS group than in the UP group(P<0.01). Conclusions:TACS appears to be a simple,effective and quick method in achieving homeostasis in women with CPP during cesarean delivery.

    Clinical Study of the Efficacy Therapy for Persistent Ectopic Pregnancy

    Objective: To investigate the efficacy therapy for persistent ectopic pregnancy(PEP). Methods: 155 patients with persistent ectopic pregnancy were randomly divided into 3 treatment groups: Group A received expectant treatment, Group B received methotrexate local injection treatment, and Group C received systemic administration mifepristone and methotrexate. Successful rate, failure rate, adverse reaction, treatment time and treatment cost of each groups were analyzed and compared. Results: The successful rate of Group B is highest. The failure rate of Group A is highest. The side effect in Group C is strongest, less side effect in Group B. Longer treatment time in Group A, shorter treatment time in Group C, there is no statistical differences in the time of 3 groups. The cost of Group C is highest, and Group B is lowest if the treatment is successful. Conclusions: There are a lots of treatment for persistent ectopic pregnancy, Methotrexate local injection treatment is the best therapy for persistent ectopic pregnancy.

    Objective:To investigate the value of conventional Papanicolaou smear (Pap) improved by take-samples in screening cervical carcinoma. Methods: Conventional Pap smear by improved take-samples, liquid-based cytology test, hybrid capture Ⅱ human papillomavarus test were performed in 500 women of 30-59 years old without screening cervical cancer in 3 years in Shang Mei-lin community of Shenzhen Futian region from December 2007 to March 2008. We obtained samples of Pap by a broom-type sampling device(take-samples used for LCT) with split-sampling method. The positive threhold of cervical cytology was atypical squamous cells of undetermined sign(ASCUS). The women with either testing result of Pap,LCT or HPV positive received multi-spot biopsy under colposcopy. The pathology results were the final diagnosis criteria to evaluate the value of two cervical cytology for screening cervical cancer. Results:The best screening cervical intraepithelial neoplasia (CIN)Ⅱ threhold of cervical cytology was ASCUS;There were no significant differences of screening effectiveness and unsatisfactory rate between Pap with improved take-samples and LCT; The complete concordance of diagnosis between Pap and LCT was 86.8%,The higher grade cervical lesion,the better concordance of diagnosis between Pap and LCT. Conclusions:Pap smear with improved take-samples is economic and effective and is worth popularizing in screening cervical carcinoma.

    Abdominal Wall Endometriosis: a Review of 65 Cases

    Objective:To explore the clinical characteristics, treatment as well as prevention of abdominal wall endometriosis(AWE). Methods:65 cases of AWE treated at Tianjin Central Hospital of Obstetrics and Gynecology from year 2008 to 2009 were reviewed. The average age was(30.7±3.6)years,range from 23 to 37 years. All of whom undergone the first cesarean section presented with pain and mass of cesarean scar 8 to 36 months after surgery,were hospitalized 9 to 72 months after surgery. The size of mass was from 1 cm × 2 cm to 4 cm × 4.5 cm. The characteristics of mass include unclear edge,solid,hard,tenderness and inmovable. In 23 cases,CA125 were mildly elevated. Results:All the cases were secondry to cesarean section. Of 65 cases with AWE,49 were located in adipose layer,13 cases were as deep as rectus abdominis. Peritoneal were invaded in 3 cases. All cases were received surgical treatment while in 5 cases which lesion diameter > 4 cm were given gonadotropin releasing hormone agonist(GnRHa) treatment for 3 times(per 28-32 day)prior to surgery. No recurrence was observed during 4-20 month follow-up and postoperative administration for 3 months. Conclusions:AWE could be diagnosed according to its medical history,typical clinical manifestations as well as gynecological check-up. Surgical treatment is the preferred choice. A complete excision with clear margin is very important. Medicine treatment after operation can prevent recurrence. Appropriately preventive measure is important during cesarean section.

    GPⅡb/Ⅲa receptor is the most redundant glycoprotein on platelet membrane, and palys a critical role in the process of platelet aggregation and thrombosis. Preeclampsia is a pathogestation with polysystem imbalance, characterized with hypertension, proteinuria, hypercoagulation, hyperfibrinolysis tendency, and is associated with high maternal high morbidity and mortality and parturient cardiovascular and cerebrovascular disorders in long term. GPⅡb/Ⅲa receptor is a key component in the process of preeclampsia invasion, which could provide original breakthrough for its etiopathogenisis and clinical therapy.

    interfering condition from RBCs on the umbilical cord blood,the rate of cells′ enrichment by discontinuous density
    centrifugation on different densities were determined by Flow Cytometry(FCM). Results: The average purity of NRBCs
    enrichment in the umbilical cord blood could take from 2.4% to 9.7% (u=3.19, P<0.05) by interfering, which means to increase
    about 4 times of enrichment of NRBC. Conclusions: Both Verapamil and Furosemide could promote the rate of NRBC′enrichment
    about 4 times.

    Soluble endoglin(sEng, sCD105)is a 65 ku serum protein and its aminoacid sequence corresponds to N-
    terminal soluble structure of full-length endoglin. It may be shed by membrane type matrix metalloprotease-1(MT1-MMP). The
    serum level of sEng was elevated in preeclamptic individuals and correlated with disease severity and gestational weeks.
    Besides, it was elevated 6-10 weeks before the onset of preeclampsia. sEng blocks transforming growth factor (TGF)-β1-
    mediated formation of human umbilical vein endothelial tubes in vitro and induces vascular permeability and hypertension in
    pregnant rats in vivo. The research of the effect of sEng in preeclampsia is one of the recent progresses in the pathogenesis
    of preeclampsia.

    The Action of Chemokines in the Pathogenesis of Endometriosis

    Endometriosis is one of the common gynecological diseases defined by the presence, growth, and invasion of endometrial tissue outside the uterine cavity. It is mainly associated with chronic pelvic pain, dysmenorrhe and infertility and its prevalence increases year by year. Although many hypotheses have been proposed, the etiology of the disease remains an enigma. Inflammation and dysimmunity are important pathologic basis of endometriosis, Attachment-aggression-angiogenesis is the basic step of occurrence and development of EMs. Recent studies have suggested that the chemokines are involved in cultivation of endometriosis in the course of chemokine, adhesion, invasion, angiogenesis and cell proliferation and apoptosis and other important links. In this article, we make the review on the roles of chemotactic factor in occurrence and development of EMs.
