    Research Progress of miR-124 in Gynecological Malignant Tumors

    MicroRNA (miRNA) are endogenous single-stranded noncoding RNA, which generating the role of transcriptional silencing by complementary binding to the target genes. In recent years, a large number of studies have shown that the occurrence of wide variety of tumor associated with abnormal expression of miR-124, which plays an important role in inhibiting of cell proliferation, promoting apoptosis and inhibiting the invasion and metastasis of tumor. Researches have revealed that miR-124 is remarkably down-regulated in many tumor cells and ontic tissues, and is closely related to the occurrence and development of tumor. In recent years, the study of the role of miR-124 in gynecological malignant tumor is increasing, but its mechanism still need further research. This article elaborates biological function of miR-124 and improvements of studies in gynecological malignant tumors.

    Progress of the Correlation about Matriptase and the Invasion and Metastasis of Endometrial Cancer

    Invasion and metastasis is one of the important biological features of malignant tumor, which leads to the cancer patients deaths. Tumor invasion and metastasis is a complex pathological process involving many kinds of protease which including metal matrix protease, serine protease and so on. Recently, we found a type Ⅱ transmembrane serine protease named Matriptase which can enhance the process of tumor invasion and metastasis through activating urokinase-type plasminogen activator, hepatocyte growth factor/scatter factor, protease activated receptor 2 and prostate. It was closely related to the occurrence and development of a variety of malignant tumors, such as breast cancer, esophageal cancer, prostate cancer, cervical cancer, ovarian cancer and endometrial cancer. Thus, Matriptase is expected to become a new biomarker for tumor diagnosis and a promising treatment target. Now we will discuss the correlation about Matriptase and the invasion and metastasis of endometrial cancer.

    Research of the Correlation between Lipid Metabolic and Endometrial Cancer

    In plasma lipids are collectively referred to as lipid, including triglycerides, phospholipid, cholesterol and the ester, free fatty acids, etc. Lipid metabolism disorder is generally elevated triglycerides(TG) and low high-density lipoprotein cholesterol(HDL-C). Since leptin found in 1994, people begin to discover the roles except the traditional ones of lipid metabolism(such as energy, synthetic hormones, etc.). Many studies suggest the correlations between lipid metabolism and cardiovascular diseases, malignant tumors. Researchers found that elevated triglycerides in the blood plasma associated with the pathogenesis of endometrial carcinoma, however, the study has not yet found that low HDL-C is one of the pathogenesis of endometrial cancer risk factors. The current studies have found the correlations between lipid metabolic disorder and endometrial cancer. Lipid metabolic may effect on endometrial cancer by insulin resistance that plays an significant role in endometrial cancer development. Studies have found that lipid metabolism related substances, such as leptin, sterol regulating element binding proteins(SREBPs), peroxidase body growth activated receptor(PPAR), may be related to endometrial cancer.

    MicroRNA and Its Role in Ovarian Cancer

    MicroRNA(miRNA) is a type of small evolutionarily conserved non-coding RNAs of 21-23 nucleotides in length that act as post-transcriptional regulators which involves in fundamental cell processes,such as,differentiation,proliferation and death. Recently,it has become clear that alterations in the expression of miRNAs contribute to the pathogenesis and progression of several human malignancies,such as colorectal cancer, cervical cancer, ovarian cancer, and so on. MiRNAs dysregulated expression plays an important role in ovarian cancer pathogenesis,metastasis, resistance and prognosis. They might be used as a noninvasive diagnostic and prognostic biomarker in ovarian cancer,and provide a new clinical thought for the analysis of chemotherapy drug resistance and individualized therapy. This article reviews the functions of miR-200 families,let-7 families and miR-119a, which are frequently dysregulated in ovarian cancer.

    The Human Epididymis Protein 4 Detection of Early Ovarian Cancer

    Ovarian cancer threatens women′s lives and health which is one of the three malignant tumors of reproductive tract, its younger trend obvious and mortality rate ranked first. It is a key point to reduce the mortality rate of the patients by improving the detectable rate of the ovarian cancer at early stage. However, the traditional ovarian tumor markers carbohydrate antigen 125 (CA125) which is applied widely, shows a low sensitivity and specificity in ovarian cancer and also appears high level in benign gynecological diseases nonspecifically, therefore, a higher sensitivity and specificity detection index is needed urgently. In recent years, the serum marker human epididymis protein 4 (HE4) has become the focus of the early diagnosis of ovarian cancer. Compared to CA125, HE4 were not expressed in most of the ovarian malignant tumor or low expression, and detection of HE4 alone or combined CA125 in early diagnosis of ovarian cancer, curative effect and prognosis evaluation of an edge. Now HE4 has been applied in foreign and some first-class hospital in China. This paper reviews the discovery of HE4, development and clinical application and ovarian malignant tumor with CA125 constructs the risk of ovarian malignancy algorithm(ROMA).

    Chemotherapy is the main treatment for immune diseases and cancer, but chemotherapy drugs may cause ovarian dysfunction in female. Amenorrhea, infertility, vasomotor symptoms, sexual dysfunction, osteoporosis and other problems caused by ovarian dysfunction bring great suffering to the patients. These also impact on patients′ psychosocial and sexual psychology. It is very important to find relevant prevention measures to reduce ovarian damage and meantime retaining reproductive and endocrine function in patients. This article summarize the relevant literature, the protection of ovarian function and fertility methods, for example, GnRH analogues, embryo cryopreservation and oocyte cryopreservation, ovarian tissue cryopreservation, apoptosis inhibition, free radical scavenger, therapy of traditional Chinese medicine are described.

    Advance of Tumor Microenvironment in Cervical Cancer

    Cervical cancer is one of the most common malignancy of women,which has caused a serious threat to the health of women. The tumor microenvironment of cervical cancer consists of cancer-associated fibroblasts,tumor associated macrophages,immune cells,extracellular matrix and other ingredients. For HPV positive cervical cancer,one of roles of HPV in promoting cervical carcinogenesis was that HPV activated IL-6/STAT3 signaling. Tumor-associated fibroblasts and tumor-associated macrophages,which are two important cells in the tumor microenvironment,can secret many cytokines and interact with several constituents of tumor microenvironment,and thus facilitating the genesis,development and metastasis of tumor. Moreover,various immune cells,namely Langerhans cell,helper T cell,cytotoxic T cell,regulatory T cell and components of extracellular matrix,such as MMP,TGF-β,VEGF consist in the microenvironment of cervical cancer as well,exercising a great influence on the genesis and development of cervical cancer. By studying the microenvironment,it will help us to further understand the mechanism of development and progression of cervical cancer,which thus provides the basis for clinical diagnosis and treatment of cervical cancer. The paper reviews recent advances of tumor microenvironment in cervical cancer.

    The Progress of Autophagy Involved in Cervical Cancer

    An increasing number of studies show that the change of autophagy level in normal cell have a dual role in influencing the development of tumor and the efficacy of antitumor drugs. Cervical epithelial cell autophagy activity changing impact on the occurrence and development of cervical cancer is uncertain, and the relationship between autophagy and cervical cancer chemotherapy drug research is less autophagy activity in cervical epithelial cell impacting on the occurrence and development of cervical cancer remains to be determined. In addition, there were a few researches report the relationship between autophagy and the effect of chemotherapy drugs. Reviewing the latest research reports about the effect on autophagy occurrence and development of cervical cancer and the related research of the interaction relationship between autophagy and cervical cancer chemotherapy medicine, to further understand autophagy plays an important role in the development and treatment of cervical cancer, which provide a reference direction for the new therapy of cervical cancer.

    Research Progress of the Folate Receptor Alpha in Gynecological Malignant Tumor

    The folate receptor(FR) is a glycosylphosphatidylinositol linked protein, including FRα, FRβ, FRγ. Its expression is low in normal tissue except for higher in human tumor cell membrane surface in the development of tumor. In recent years, studies have shown that FRα has more expression in adenocarcinoma, such as ovarian cancer, endometrial cancer, cervical adenocarcinoma, non-small-cell lung cancer and malignant mesothelioma. Researches have shown that FRα affects the occurrence and development of tumor by participating in folic acid metabolism and estrogen receptor regulation and ERK signaling pathway. FRα expression in the gynecologic tumor is still controversial, and the specific mechanism is unknown. The study of various mechanisms of FR will be helpful in the early diagnosis and treatment of gynecological tumor.

    The Down-Regulation of miR-199a in Ovarian Cancer and Its Effect on Ovarian Cancer Biological Functions

    Objective: To study the expression and potential function of miR-199a in ovarian cancer occurrence and progression. Methods:Quantitative reverse transcriptase PCR(qRT-PCR) was used to detect the expression of miR-199a in 20 cases of epithelial ovarian cancers, 15 cases of benign epithelial ovarian tumors, ovary normal epithelial cell (IOSE) and ovarian cancer cells. Retrovirus expression vector pMSCVpuro-miR-199a was constructed and SKOV3 cells were transfected. CCK8 and transwell methods were used to detect the ability of proliferation and metastasis in SKOV3 cells after stably transfected with pMSCVpuro-miR-199a. Results:The mRNA expression of miR-199a in epithelial ovarian cancer group was significantly lower than that of in benign ovarian tumors (P=0.001). Compared with IOSE cells,the miR-199a expression were down-regulated in ovarian cancer cells and expecially in SKOV3 cells(P<0.01). The abilities of proliferation,migration and invasion in SKOV3 cells transfected with pMSCVpuro-miR-199a were significantly lower than that in pMSCVpuro-NC SKOV3 cells(P<0.000 1). Conclusions:Our data suggest that the expression of miR-199a was down-regulated in ovarian cancer and miR-199a might play as an anti-oncogene in ovarian cancer proliferation and metastasis.

    Objective:Investigationg of Huanggang Tuanfeng county and Xishui county rural women cervical cancer prevention and treatment knowledge popularization, dicusses the methods and measures of improve the level of the city′s cervical cancer cognitive. Methods:Through the countryside Tuanfeng county and Xishui county of Huanggang city women cervical cancer prevention knowledge questionnaire issued, statistics aware of knowledge about prevention and control, access to cervical cancer prevention and treatment knowledge and refused to take part in the cause of cervical cancer census. Results:Old age, low degree of culture, low household income, marital instability, the floating population, women with reproductive tract infection disease always without as low awareness, Tuanfeng county and Xishui county general awareness of cervical cancer prevention knowledge rate respectively is 51.4%, 35.6%. The awareness rate of Tuanfeng county is higher than Xishui county (P<0.01). By the hospital, community lectures, public health publicity column and brochures and doctors face-to-face education proportion of acquiring knowledge were 30.9%, 33.2%, 27.0%, respectively. No symptoms don′t want to do after check and find out the psychological burden proportion were 50.0% and 27.7% respectively. Conclusions:Should be targeted on the low awareness and further strengthen health education in remote areas, through lectures, hospitals, community public health publicity column and brochures and the doctors face-to-face education expanded publicity.

    The Prevention of Thromboembolism Gynecologic Cancer Patients during Perioperative:A Systematic Review

    Objective: This study was designed to systematically analyze the world′s literature on thromboembolism prophylaxis in gynecologic oncology perioperative and evaluate the efficacy and safety of prophylaxis regimen. Methods:Computerized searches electronic databases such as Pubmed,Cochrane Library,Medline,Embase and CNKI,VIP,CBM from the date of their establishment to Sep 2014. Meta-analysis was performed on the studies meeting the criterion. Results:It included 18 randomized controlled studies and 3 193 patients. Meta analysis showed that:①The analysis of heparin-versus-control revealed a significant decrease in DVT in patients receiving heparin(RR=0.59,95%CI:0.36-0.96),but not in PE(RR=4.08,95%CI:0.71-23.53).②LMWH-versus-control also revealed a significant decrease in DVT in patients receiving LMWH(RR=0.16,95%CI:0.05-0.47). ③In the heparin-versus-LMWH studies,there was no significant difference(RR=0.98,95%CI:0.43-2.23). There was no significant difference in PE in patients(RR=1.70,95%CI:0.44-6.63). ④Combination prophylaxis can be more effectively reduce the incidence of deep vein thrombosis(RR=0.15,95%CI:0.04-0.68). Conclusions:All gynecologic cancer patients should receive VTE prophylaxis. But no one prevention modality can be considered superior. Adequately powered RCTs are urgently needed to determine the optimal regimen in these high-risk patients.

    Arachidonic acid is one of the essential free fatty acids. Arachidonic acid and its products play an important role in regulating the body inflammation, blood vessels, elastic, leukocyte function, bone remodeling and platelet activation. Recent studies have found that arachidonic acid levels in the blood may change after menopause. Arachidonic acid and its product are related with postmenopausal hypertension, obesity, osteoporosis, etc. Moreover, after menopause, dietary factors can regulate the content of blood arachidonic acid. In this paper, before and after menopausal transition to the variation in content of arachidonic acid and its possible mechanism and postmenopausal women the relationship between diet and arachidonic acid in the body change were summarized.

    The silent information regulator 2 (Sir2) and family proteins are highly conserved NAD+(nicotinamide adenine dinucleotide)-dependent deacetylases, and were originally discovered in yeast. SIRT1 is the highest homology with Sir2 in mammal, is considered to be long-lived protein limited by energy to give play to the role of anti-aging. SIRT1 regulates physiological processes through histones and non-histone proteins to extend life span, such as adjusting the energy metabolism, lipid oxidation and delay the onset of old age diseases such as cardiovascular development, and anti-aging. Studies have shown that estrogen plays a vital role in anti-aging. Conventional viewpoints suggested that estrogen combined with the estrogen receptor (ER) in directly or indirectly to activate the target genes in anti-aging. However, recent research shows that estrogen up-regulated SIRT1 by multiple pathways and delayed cell senescence. This paper discuss the anti-aging mechanism of estrogen by summarizing the expression pathways of estrogen up-regulating SIRT1.

    Research progress of the etiological and therapeutic mechanisms of endometriosis (EMs) are still not very clear. Well established animal model which created an appropriate method of evaluation is crucial to study the etiology and experiment treatment effect of EMs. In recent years, building simple non-invasive models with fluorescent transfection method has become a new direction to establish EMs model. Considering internationally on this method with its gradual progress of establishment in vitro and in vivo, selecting transfection process, fluorescent materials and color, quantitative and dynamic observation and measurement instrument in these years, it will be useful for the summary on these aspects of the work in the EMs animal models.

    Effect of Estrogen on Telomerase Activity in Ovariectomized Rats

    Objective:The aim of this study was to investigate the anti-aging effect of oral estrogen on the telomerase activity in ovariectomized female Sprague-Dawley(SD) rats. Methods:Thirty-three three-month-old female rats were randomly divided into ovariectomized experimental group (Treated, n=11), ovariectomized control group (OVX, n=11) and sham-operated group(Sham, n=11). The rats in the Treated group were given 0.21 mg/kg estradiol(E2) valerate intragastric administration while other two groups were given the equal physiological saline intragastric administration. All of the animals were euthanized 12 weeks after treatment, and abdominal aortic blood samples were taken to assess the level of E2, follicle stimulating hormone (FSH) by ELISA. Telomerase activity and telomerase reverse transcriptase(TERT) mRNA expression in the heart, liver, brain tissues of all rats were measured by TRAP-ELISA and RT-PCR, respectively. Results:The serum E2 level in the Treated and Sham group were significantly higher than that in the OVX group(P<0.05). The serum FSH levels were significantly lower than that in the OVX group (P<0.05). Compared to the OVX and the Sham group, the value of ΔA on telomerase activity in the Treated group was significantly increased(P<0.05). The telomerase activity was negative and ΔA value was low in the OVX and Sham group, no statistical difference between them(P>0.05). The value of TERT mRNA ΔCt in the heart, liver and brain tissues in the Treated group were significantly lower than those in the OVX and Sham group(P<0.05). But there was no significant difference between the OVX and Sham group(P>0.05). Conclusions:Estrogen can significantly up-regulate telomerase activity and TERT mRNA expression in the ovariectomized female SD rats to delay senescence.

    The Relationship between Age, Menopausal Status and Homocysteine Level

    Objective: To investigate the relationship between age, menopausal status and homocysteine levels. Methods:We collected 450 patients from Beijing Friendship Hospital gynecological clinic (123 patients were in menopausal transition period and 327 patients were postmenopausal period). Patients aged 45-65 years old never used HRT were measured homocysteine levels in fasting venous blood. Results:Of the 450 patients, the homocysteine level was an average of (8.4±3.1) μmol/L, 5.7~45.4 μmol/L. Without considering the menopausal status and menopausal years, before and after stratification by the body mass index, homocysteine level in different age groups showed significant differences (P<0.05). Without considering age, the homocysteine levels were not significant differences between different menopausal status and menopausal years (P>0.05). After adjusting the age and BMI, the homocysteine levels were not significantly different between different menopausal status and menopausal years (F=30.67,P>0.05). Conclusions:Without considering the menopausal status and menopausal years, age is the main factor affecting homocysteine level of women, stratificated by the body mass index, age still affects homocysteine level significantly. After adjusting the age and BMI, the homocysteine levels were not significant different between different menopausal status, homocysteine level did not change with the menopausal years.

    Expression and Clinical Significance of CD147,MCT1 and MCT4 in Cervical Lesions

    Objective: To study the expression and clinical significance of CD147, monocarboxylate transporters 1 (MCT1) and MCT4 in cervical lesions and cervical cancer. Methods:Choose 92 patients with cervical lesions, including 43 cases of carcinoma of the cervix (cervical cancer group), 34 cases with CIN(CIN group, 12 cases of CINⅠ, 11 cases of CINⅡ,11 cases of CINⅢ), 15 cases of normal cervical as controls (control group). Immunohistochemistry was used to test and analyze the expression of CD147, MCT1 and MCT4. Results:The positive expression rates of CD147, MCT1 and MCT4 in cervical cancer group was higher than that in CIN group (all P<0.05) and control group (all P<0.05). CIN group was higher than control group(all P<0.05), CINⅡ-Ⅲ group was higher than CINⅠ group (all P<0.05). In cervical carcinoma group, the positive expression rates of CD147 and MCT1 in age≤35 years old group was lower than that in the group of age>35 years old group, that of stage Ⅰ-Ⅱ group is lower than stage Ⅲ-Ⅳ, and that of high differentiation group was lower than that in low differentiation group (all P<0.05). In cervical carcinoma group, the positive expression rates of MCT4 in aged≤35 years old group was lower than that in the group of age>35 years old group, and that of high differentiation group was lower than that in low differentiation group (all P<0.05). Differences between stage Ⅰ-Ⅱ group and stage Ⅲ-Ⅳ group had no statistical significance. There was statistically significant consistency between the expression of MCT1 and CD147 (weighted Kappa=0.774,Z=10.126,P<0.001). There was statistically significant consistency between MCT4 and CD147 (weighted Kappa=0.909,Z=11.628,P<0.001). There was statistically significant consistency between MCT1 and MCT1 (weighted Kappa=0.797,Z=10.383,P<0.001). Conclusions:The expression of CD147, MCT1, MCT4 were consistently higher level in cervical lesions and play important roles in the development of CIN and cervical carcinoma. They can be accepted as the objectively high risk indexes in progression from low level to high level of CIN and cervical cancer. Low and high level CIN with high expression of CD147, MCT1 and MCT4 should be given active treatment and followed-up.

    Objective: By detecting collagenⅠ,collagenⅢ and elastin′s expression in pelvic ligament tissue,we can investigate the effect and possible mechanism of pueraria in women with pelvic floor dysfunction(PFD). Methods:Select 60 menopause patients with PFD who had underwent vaginal hysterectomy from December 2010 to November 2012. Based on POP-Q score,30 patients in Ⅰ-Ⅱ degrees defined as mild group,30 patients in Ⅲ-Ⅳ degrees defined as severe group. 30 patients in the treatment group took pueraria in 60 days before the surgery including the 15 patients in Ⅰ-Ⅱ degrees and the rest in Ⅲ-Ⅳ degrees. The control group including 30 patients,15 cases in Ⅰ-Ⅱ degrees and 15 cases in Ⅲ-Ⅳ degrees,did not take any exogenous estrogen. We use HE staining to observe the form of collagen and Weigert staining to observe those of elastin. Then we use the immunohistochemical two-step method and image analysis to investigate the expression of collagen Ⅰ,collagen Ⅲ and elastin in uterosacral ligament tissue. Results:①Collagen and elastin are both widely expressed in uterosacral ligament tissue through HE and Weigert staining from light microscopy,and their staining expression in the treatment group are deeper and more even,the number are richer, and more intensive distribution than in the control group. ②In the control group,the expression of collagen Ⅰ,collagen Ⅲ and elastin in mild group ligament tissue are higher than those in the severe group(P<0.01). ③In the treatment group,the expression of collagen Ⅰ,collagen Ⅲ and elastin in mild or severe group ligament tissue are higher than those in the control group(P<0.01). Conclusions: The pueraria can increase the expression of collagen Ⅰ,collagen Ⅲ and elastin in the pelvic tissue,thereby enhancing the supportive effect of pelvic floor tissue which can relieve the symptoms of PFD. Puerarin as an effective drug for the treatment of PFD provides a possible theoretical basis.

    Explore the Relationship between Endometrial Polyp Characteristics and Abnormal Uterine Bleeding and the Influence to Its Malignancy

    Objective: To evaluate the relationship between endometrial polyp(EP) characteristics and abnormal uterine bleeding, and analyze the influence to its malignancy. Methods:Retrospectively collect cases from January 2010 to June 2013 who received hysteroscopy and confirmed diagnosis as endometrial polyps in our hospital. Review and summarize polyp number and size of each case, and analyze the relationship between them and abnormal uterine bleeding. By pathology results, the polyp patients were divided into benign polyp and potentially malignant or malignant polyps. Analyze the relationship between the various risk factors associated with malignant polyps and malignant potential. Results:Of the included 754 cases, 38.59%(291/754) of patients with bleeding symptoms. Probability of multiple polyps and solitary polyp with bleeding were 44.26%(135/305) and 34.74%(156/449), and the difference was statistically significant(χ2=6.94,P<0.01). In 449 patients with solitary polyp, probability of large(size>10 mm) and small polyp(size <10 mm) with bleeding were 38.69%(106/274) and 28.57%(50/175), and the differene was statistically significant(χ2=4.82,P<0.05). Among polyp patients, 2.92%(22/754) of precancerous lesions, 3.18%(24/754) of cancerous. By Logistic analysis, the factors of postmenopause, polyps diameter>10 mm, multiple polyps and merging bleeding symptoms have a significant influence on precancerous and cancerous polyps(P<0.01). Conclusions:Bigger(>10 mm) or multiple polyps may be the risk factors and have a higher probability of abnormal uterine bleeding manifestations. Menopause,polyp diameter>10 mm, multiple polyps, abnormal uterine bleeding are risk factors for malignant polyps.

    Clinical Study of Patients with RVVC and Intestinal Candida Yeast Infection

    Objective: Further study to the clinical study on the treatment of recurrent candida vulvovaginitis and intestinal homologous candida infection. Methods:Collect and identify vulva vaginal secretion and anal swab strains of recurrent candida vulvovaginitis patients in June 2009 to March 2013 in obstetrics and gynecology clinic, for double-positive patients, determine the homology of candida mycoderma bacteria got by separating secretion and anal swab strains by the method of PCR, divide the patients into two types of homologous patients and non homologous patients. Two types were respectively assigned to four groups randomly. Group A (homologous systemic treatment group) and group B (non homologous systemic treatment group) were treated with oral fluconazole, group C (homologous local treatment group) and group D (non homologous local treatment group) were treated with external using Miconazole Nitrate Suppositories at least 6 months. And the patients of group A and B should be consolidation therapy at least 6 months. Follow up 1 year after four groups patients recovered (Curative effect evaluation in the result of candida microscopic examination and culture were negative), to observe the relapse of four groups patients. Results:Total 160 patients were enrolled in RVVC patients, covered 110 cases of RVVC homology patients and 50 cases of patients with non homologous. Follow up the treatment effect, group A showed that the recurrence rate of the patients with a minimum of 10.7%, recurrence rate of C, D are high with 70.4% and 69.2%. In the fact of first time to relapse and recurrence degree, group A is better than the other three groups, while group C and D are relatively worse. In addition, homology test of double positive and part of double positive patients with recurrent showed that the effect of systemic treatment is better than local treatment. Conclusions:Further confirmed that strains transition between recurrent candida vulvovaginitis and intestinal homologous candida infection probably has the close relationship with the reason why part of RVVC patients last long illness.

    Exploration of the Relationship between Thyroid Dysfunction and Missed Abortion

    Objective: To investigate the correlation between thyroid dysfunction and missed abortion. Methods:We choosed 95 patients diagnosed missed abortion whose gestational age less than 12 weeks as observation group, while 92 patients who have normal pregnancy as control group. Chemiluminescence method was used to detect the serum levels of thyrotropin hormone (TSH), free thyroxine(FT4) and thyroid peroxidase antibody(TPOAb) in the two groups. Results:Comparing the incidence of hyperthyroidism, hypothyroidism and low thyroxine in both group, the difference was not statistically significant (P>0.05). There were statistical differences of subclinical hypothyroidism and positive TPOAb incidence rate between observation group and control group(P<0.05), and they have weak correlations with missed abortion (correlation coefficients were 0.21 and 0.22). Conclusions:There is correlation between thyroid dysfunction and missed abortion, the incidence of missed abortion is increased in subclinical hypothyroidism and positive TPOAb, so we should carry out the relevant checks thyroid function in pregnancy to reduce the incidence of missed abortion.

    The angiopoietins(Angs) are a novel family of newly-discovered angiogenic factors which are secreted by vascular endothelial cells, plays an important role in regulating angiogenesis, degeneration and vessel stabilization. Ang-1 and Ang-2 participate in the development of embryonic blood vessels, as well as the angiogenesis and metastasis of tumors. Embryo implantation is similar with tumor during pregnancy, therefore, Angs and the tyrosine kinase Tie-2 receptor reside in both placenta and fetus, the expression of Angs during pregnancy may represent a feature for embryo implantation, placental angiogenesis, as well as the growth of the fetus. Hence, in the process of pregnancy, if anyone link in angiogenesis barrier, will cause the failure of embryo and the occurence of pregnancy complications. Therefore, Angs for further study of the role of pregnancy, will help to gestational disease diagnosis and treatment.

    The umbilical cord is an important connection between the fetus and placenta tissue,is maternal and fetal material exchange link. In recent years,a large number of domestic and international studies have shown that abnormal umbilical cord morphology can cause fetal circulation problems leading to the occurrence of fetal and neonatal severe adverse outcomes,such as fetal growth restriction,fetal distress, neonatal asphyxia or death. The fatality rate of abnormal umbilical cord neonatal was 28.5% reported previously, Therefore,correct understanding of abnormal umbilical cord is very important. In this paper,the relationship between morphological abnormal umbilical cord and fetal outcome is reviewed. And abnormal umbilical cord length,umbilical cord entanglement,umbilical cord twist,umbilical cord knot,abnormal umbilical cord attached,single umbilical artery,and so on, all can affect fetal and neonatal adverse outcomes.

    The Effect of Recombinant IL-17 Administered in the Vaginal Vaults on Normal Pregnancy CBA/J Mice

    Objective: To investigate the effect of transvaginal injected recombinant IL-17 on the pregnancy outcome in the CBA/J mice model (CBA/J×BALB/c) and its role in feto-maternal immune rejection. Methods:Six to eight-week-old female CBA/J and male BALB/c were divided into three different experimental groups. Mice of normal pregnancy(CBA/J×BALB/c) received no treatment were served as control group. 50 μL phosphate-buffered saline(PBS) or 50 μL recombinant IL-17 (rIL-17) was administered in the vaginal vaults of anesthetized pregnant CBA/J mice on Day 1 of pregnancy served as PBS group and rIL-17 group individually. The numbers of surviving and reabsorbed fetuses in each group were counted, and the expression of TGF-β and IL-10 in decidua was assessed by quantitative real-time PCR and western blotting on Day 14 of pregnancy. Results:In this in vivo model of normal pregnancy, exogenous rIL-17 resulted in increased spontaneous resorption of fetuses and decreased expression of TGF-β and IL-10 (P<0.05). And there was no statistical difference between PBS group and control group (P>0.05). Conclusions:Intravaginal rIL-17 can induce inflammation in feto-maternal interface resulting in abortion in normal pregnant CBA/J mice model.

    B-Lynch Suture in Application of Hemorrhage in Cesarean Section Postpartum

    Objective: Discuss the effect of B-Lynch suture technique to postpartum hemorrhage on cesarean section. Methods:80 patients had been subjected to cesarean section with postpartum hemorrhage. They were selected and divided into observed group(40 cases) and controlled group(40 cases). The observed group was received B-Lynch suture technique, the latter was subjected to the traditional methods of hemostasis. Both of efficiency, intraoperative hemorrhage, operation time, cost of hospitalization were compared. Results:Compared with the control group, the observation group of less intraoperative and postoperative blood loss,shorter hospitalization time, hospitalization expenses low, differences were statistically significant(P<0.01). And good postoperative recovery, no complications occurred. Conclusions:B-Lynch suture is an effective method of treatment of postpartum hemorrhage in cesarean section, is worth promoting.

    Expression and Significance of miRNA-203 in Endometrial Decidualization

    Objective:To detect the expression of miR-203 in endometrium and decidua, and explore its significance during decidualization of endometrium. Methods:14 secretory endometria, 16 early pregnancy deciduas and 18 spontaneous miscarriage deciduas were collected in the study. MiR-203 expression level was detected by quantitative polymerase chain reaction(qPCR). And IGF2R was predicted as the target gene of miR-203 respectively. Furthermore,the miRNA expression of IGF2R was detected by fluorescence quantitative PCR. Results:The expression of miR-203 in secretory endometrium was higher than that in spontaneous miscarriage decidua and early pregnancy decidua (P<0.05). Moreover,IGF2R was increased in early pregnancy,comparing with secretory endometrium and spontaneous miscarriage decidua(P<0.05). Conclusions:The expression of miR-203 may be associated with decidualization by targeting IGF2R. And IGF2R might associate with early pregnancy maintenance.

    Analysis of 70 Cases with Malformations in the Fetuses of Twin Pregnancy

    Objective:To investigate the clinical analysis from 70 cases with malformations in the fetuses of twin pregnancy from 2011 to 2013 at Shenzhen Maternal and Child Healthcare Hospital. Methods:Describe the clinical characteristics of 70 twin pregnancies complicated with fetal malformations. According to the way of conception and the type of chorionic,these twin pregnancies were divided into natural conception and artificial insemination group, monochorionic and dichorionic group respectively, and were compared the pregnancy outcomes and the main clinical characteristics by different groups. Results:65.71% of pregnant women were equal or greater than 30 years old, and 64.29% are monochorionic twins. Pregnancy complications were remarkably higher(91.42%), the top five complications were preterm labor, weakness of contractions, placental abnormality, selective growth restriction, hypertensive disorders in pregnancy. One fetus affected was 89.33%, the top five abnormalities were cardiovascular system, musculoskeletal system, urogenital system, cleft lip and palate and nervous system. Conclusions:Twin pregnancies complicated with fetal malformations are above 30 years old, and have higher incidence of complications. One fetus affected is more common than both fetuses affected; single defect is more common than multiple defects. A single system malformation is more common than multiple system malformation. The cardiovascular system malformation is the most common malformation in twin fetuses, musculoskeletal system is the second and the urogenital system malformation ranks the third. Especially in ventricular septal defect in cardiovascular malformation. So, for over 30 years old pregnant women with twins and pregnant complications, we must strengthen antenatal fetal malformation screening, and especially should focus on screening cardiovascular system, bone of limbs, urogenital system malformation.

    Telbivudine Treatment of 134 Cases of Pregnant Women with Chronic Hepatitis B

    Objective:Study was performed on the safety and efficacy of telbivudine in blocking HBV intrauterine infection of fetus in late pregnancy. Methods:In 134 cases with chronic hepatitis B is that of women in late trimester of pregnancy of 22-32 weeks when they visited our hospital, based on the wishes of patients. 134 cases were divided into treatment group(n=86, treated with oral telbivudine antiviral therapy) and control group(n=48, given with compound glycyrrhizin tablets treatment). After the full-term babies delivered the umbilical cord blood were taken for testing of HBsAg, within 24 h for neonatal hepatitis B vaccine injection 10 μg and hepatitis B immune globulin 200 IU, at the same time sequential administration of hepatitis B vaccine immunization. Results:Patients′ age, HBV DNA quantitative, gestational weeks and liver function indexes (ALT, AST) had no significant difference before the treatments in the two groups(P>0.05). After treatment, HBV DNA quantitative, liver function indexes were lower than those in the control group when the pregnancies termination(P<0.05). The positive rate of HBsAg in treatment group was lower than that in the control group(P<0.05) at the birth. However, the mode of delivery in patients of the 2 groups had no effect on neonatal hepatitis B infection rate(P>0.05). Different mode of delivery of neonatal HBV infection had no significant difference after one year(χ2CMH=0.067 8,P=0.794 5). Conclusions:The treatment of oral telbivudine blocking HBV intrauterine infection is safe and effective, and is worthy of further promotion.

    Cervical cancer with definite pathogenic factors exits longer and reversible premalignant lesions which can be intervened by medical methods to decrease the morbidity and mortality. With the development of cervical cancer screening, more and more women especially young women were found with cervical intraepithelial neoplasia(CIN). The management and intervention of precancerous lesions is an important link to prevent invasive carcinoma. Combined with American Society for Colposcopy and Cervical Pathology(ASCCP) in 2013 promulgated "2012 Updated Consensus Guidelines for the Management of Abnormal Cervical Cancer Screening Tests and Cancer Precursors", we interpretated the management of cervical precancerous lesions. The uptated guidelines of great importance to direct the clinical practice prescribed the management of CIN and highlighted the follow-up value of co-test.
