    Hysteroscopy is one of the contemporary mainstream technology of minimally invasive surgery. It has been become gold standard diagnosis method and new surgical treatment model of gynecological diseases. It is no repair chance for hysteroscopic uterine renaturation surgery. Hysteroscopic air embolism complication can cause sudden death. In order to avoid adverse events,hysteroscopist not only must go through professional training, familiar with ultrasonic intervention, distension and irrigation of uterus, anesthesia monitoring, video effect, equipment and instrument performance and operating requirements, but also master laparoscopic technology. Hysteroscopist has to pay attention and carry out preoperative pretreatment of hysteroscopic surgery, to alert and prevent serious complications of hysteroscopic surgery. Through training and exercise, doctors can be a good hysteroscopist.

    Uterine leiomyomata are the commonest neoplasms of the female reproductive system in the procreative age. In these cases operation is still a treatment of choice. Hysteroscopic,abdominal and laparoscopic myomectomy,abdominal,vaginal,and laparoscopic hysterectomy,radio frequency ablation,uterine artery embolization and focused ultrasound are now available. To date,with the advancement of surgical skills and laparoscopic instruments,clinical application of laparoscopy for uterine leiomyoma is increasingly widespread. Despite the fact that laparoscopy has proven its advantages of less trauma and ache as well as shorter hospital stay and recovery period,etc.,this technique remains controversial concerning the operative indication and procedures as well as intraoperative and postoperative complications,etc. Therefore,extensive investigation on laparoscopy for uterine leiomyoma is paramountly required. This study addresses the important aspects of the indications,contraindications,surgical procedures and complications for laparoscopic myomectomy and the operative indication and procedures for total laparoscopic hysterectomy in order to assess the clinical decision making of laparoscopy for uterine leiomyoma.

    A Discussion and Review of Laparoscopic Hysterectomy

    In the era of minimally invasive surgery,laparoscopic surgery has been proven to be a viable alternative under the same operation indications. Although the laparoscopic technology started rather late in China,it develops very quickly. The contraindication of laparoscopy becomes decreasing along with the developing of new techniques and apparatus. With the advanced development of laparoscopic techniques,laparoscopic gynaecological surgery provides a new way for minimally invasive surgery. It is effective,reliable,and valuable in clinical application and has such merits as minimal invasion,rapid recovery and extensive application. Laparoscopic hysterectomy has the characteristics of minimally invasive surgery. The advantages include less trauma,smaller abdominal incision and faster recovery. However,limitations and complications of this methods should not be overlooked,so the selection of the ideal pattern for certain patient is important. This article seeks to illustrate laparoscopic hysterectomy and analyzes the existing problems compared to other ways of hysterectomy,thus providing reliable theoretical basis and a new view for clinical management.

    Treatment Strategy of Endometriosis with Infertility

    Hormonal treatment does not improve the fecundity of infertile women with endometriosis. In stage Ⅰ/Ⅱ endometriosis,laparoscopic ablation of endometrial implants has been associated with a significant improvement in live birth rates. While in women with stage Ⅲ-Ⅳ endometriosis conservative surgical treatment with laparoscopy and possible laparotomy plus GnRH agonist may increase fertility,but there is some objection. There is no evidence that the outcome is affected by the method of ablation,either electrosurgery or laser delivery systems in laparoscopy. Patients with endometriosis-associated infertility undergoing assisted reproduction respond with significantly decreased 37% pregnancy rate comparing that of women with tubal factor infertility for IVF. Prolonged use of GnRH agonist before IVF-ET in patients with endometriosis resulted in significantly higher ongoing pregnancy and implantation rates than standard controlled ovarian hyperstimulation regimens. There are some evidence supporting the idea that surgery may negatively affect ovarian reserve and other investigator consider that negatively affect will decrease to minimal by practices accumulating and protecting ovarian tissue. Patients in stage Ⅰ-Ⅱ endometriosis with infertility should be expect to nature fecundity months after laparoscopy,and who in stage Ⅲ-Ⅳ will be suggested to get assisted reproduction. There are heated argument in laparoscopy before IVF-ET/ICSI. When recurrent of endometriosis are present,more than one time IVF after GnRH agonist treatment of severe month have better pregnancy rates than laparoscopy again. Treatment with a GnRH agonist for 3-6 months before IVF in women with endometriosis increases the rate of clinical pregnancy as negative effect of fertility in many mechanisms by endometriosis. There are some introduction in Endometriosis Fertility Index(EFI),some guideline and committee opinion from European Society of Human Reproductive and Embryology(ESHRE),American Society of Reproductive Medicine (ASRM) and Chinese Medicine Association. The treatment strategy in endometriosis and infertility of author′s ART center is introduction.

    Focus on Laparoscopy in Gynecology

    During the last three decades,laparoscopic surgery has been widely accepted in gynecologic diseases with advantages including better vision,smaller incision,faster recovery,a shorter hospital stay,milder adhesion and lower perioperative complications. The role of laparoscopy has been expanded from congenital anomalies,myoma,ovarian cyst to deep infiltrating endometriosis,pelvic floor dysfunction,and even gynecological cancer. Recent technical advances in endoscopic instrumentation and computer-controlled system have made modern-day laparoscopy manifold (i.e. robotic laparoscopic surgery,single-port laparoscopy and natural orifice transluminal endoscopic surgery). Several arguments on new gynecologic applications and those advanced techniques have been published recently. Gynecology doctors and technicians together make efforts on the development of laparoscopic sugery.

    Application Advances of Laparoscope in Gynecologic Oncology

    In the past 20 years,diagnosis and treatment of gynecological malignancies has made significant progress. Especially in recent years,with the new development of equipment which laparoscopic surgery pioneers expect,performing laparoscopic surgery in gynecologic oncology has become feasible. The laparoscopic application in the diagnosis and treatment of gynecologic malignancies has multiple benefits,including image magnification,metastatic or recurrent lesions can be found easily,at challenging areas,such as the anatomy of para-vescal space and perirectal fascia space can be clearly display. Pneumoperitoneum pressure can reduce the small blood vessels bleeding,thus reducing the length of hospital stay and recovery faster. Earlier postoperative chemotherapy or radiotherapy,can greatly reduce radiation-induced intestinal complications.

    Abnormal uterine bleeding(AUB) is a femal common disease. Endometrial ablation(EA) is one of the treatments of AUB. EA could destroy the endometrium and the superficial layer of myometrium by physical or chemical methods. When the endometrium destroyed,it will lose the potential to regenerate,then the menstruation quantity become little,even amenorrhoea and excessive uterine bleeding is avoided. Up to now,there are two generations of EA. Each generation various ways destroy the endometrium to treat menorrhagia. Though endometrial ablation is more and more safe and effective,but it still has many deficiencies,such as postoperative bleeding,infection,even pregnancy. Also,menstrual related abdominal pain,such as hematometra,post-ablation-tubal sterilization syndrome(PATSS) and still exists the risk of endometrial malignant disease. There are EA related problems need further research and the summary of evidence-based medicine.

    The Role of Hysteroscopy in the Evaluation of Infertility Women

    Congenital anomalies and acquired diseases of the uterus cavity may interfere with normal implantation and placentation that may lead to infertility and pregnancy loss. Hysteroscopy is considered the“gold standard” investigation for assessment of the uterine cavity,with the ability to treat uterine pathology in infertile patients. It also improves pregnancy outcome singnificantly. Hysteroscopy was even regarded as one of assisted reproduction technique by some authors. But the value of hysteroscopy as a routine approach in the management of infertile women is a matter of debate. Hysteroscopic tubal cannulation should be considered as a treatment option for women with proximal tubal obstruction because the treatment improves the chance of pregnancy. ART combineded with hysteroscopy requires further investigation. With the advantage of direct visualization,amplification,“see and treat”,and minimal invasion,hysteroscopy plays an more important role in infertility.

    In recent years, many exciting innovations have been developed for either diagnostic or operative
    hysteroscopy,thus expanding application of hysteroscopy in the diagnosis and treatment of endometrial diseases.This paper introduces a new diagnostic endoscopic technique for endometrial diseases-narrow-banding imaging hysteroscopy. It also discusses the therapeutical application of hysteroscopy on endometrial diseases,including the conservative treantment of endometrial cancer and atypical endometrial hyperplasia, and hysteroscopic treatment of infertility resulting from intrauterine adhesions. Finally, related evidence-based issues were discussed about the dissemination of endometrial carcinoma into the peritoneal cavity.

    Staging and Classification of Endometriosis:Current Systems and the Novel YAO Classification

    Endometriosis is a kind of common gynecologic disease,which is benign but intricate. The revised American Fertility Society(rAFS) classification system has historically beenthe only classification system and the most widely used staging system. Recently,the ENZIAN classification system,developed as an adjunct to the rAFS to describe deeply infiltrating endometriosis(DIE) and the involvement of other organs has been introduced. More recently, the Endometriosis Fertility Index (EFI) that has been validated to predict non-IVF pregnancy rates in infertility patients following surgical diagnosis and treatment of endometriosis was published. YAO Classification is a novel system that based on laparoscopic exploration and pathology. YAO Classification is better at standardizingdescription,documentation,and analysis of endometriosis,especially DIE.

    Clinical Study of Hysteroscopic Bipolar Plasma Electrode System Endometrial Resection

    Objective:To investigate the effect and the safety of bipolar electrode system during endometrial resection treatment. Methods:A total of 50 patients including dysfunctional uterine bleeding and endometrial polyps were randomly divided into two groups,one including 30 cases treated with monopolar resectoscopic surgery and the other 20 cases treated with bipolar resectoscopic surgery,all of them carries on the contrast analysis. Results:The experimental group in operation time,amount of bleeding during the operation,the volume of uterine distention medium and absorption volume,were lower than the control group, difference between the two groups is statistically significant(P<0.05),the experimental group without the occurrence of uterine perforation,air embolism,transurethral resection of prostate syndrome(TURPS),and can maintain the balance of blood electrolyte;the number of days of vaginal bleeding or discharge liquid of experimental group is less than the control group(t=4.254,P<0.001);the treatment effect of the two groups had no significant difference(χ2=0.591,P=0.807). Conclusions:The bipolar electrode system during endometrial resection is fast,safe,effective,and is worthy of further promotion.

    Study on Hysteroscopy-assisted Diagnosis and Treatment of Oblique Vaginal Septum Syndrome

    Objective:To study the clinicle value of hysteroscopy in diagnosis and treatment of oblique vaginal septum syndrome. Methods:13 patients(7 cases of typeⅠ,1 cases with congenital urethrovaginal fistula,6 cases of type Ⅲ,Ⅱ) with oblique vaginal septum syndrome(OVSS) are enrolled from 2003 January to 2011 December. A systematic retrospective analysis,age 11-20 years old,average age is 13.6±2.8 years old,with abdominal pain after menarche,periodic abdominal pain or a pelvic mass was detected on admission,preoperative hysteroscopy combined with ultrasonography can make clear the clinical diagnosis. Hysteroscopy can find total reproductive development,guide and locate the septum position. Incision and resection of oblique vaginal septum,removal of vaginal or intrauterine hematocele or empyema. Results:Postoperative dysmenorrhea symptom disappeared,the volume no change,no vaginal or abdominal mass happened again. Conclusions:In diagnosis of oblique vaginal septum, hysteroscopy is an important means, combined with ultrasound examination to clearly understand reproductive development and localize of oblique vaginal septum position,guide operation resection or clear cut parts. By hysteroscopy and ultrasonography can make oblique vaginal septum operation more accurate,safe,fast,and direct.

    Female pelvic arterial system is a huge and complex arterial system and the development of its research methods could be largely divided into two phases,autopsy and in vivo research. Due to the numbered corpses,it is almost impossible to promote a large scale study based on the data collected from corpses. For in vivo research,at present,magnetic resonance angiography and computed tomography angiography could offer a far more clear view of female pelvic artery than X-ray and ultrasound,which earn them a substantial popularity in this field of study. And the roles of X-ray and ultrasound have been gradually replaced by magnetic resonance angiography and computed tomography angiography. In recent years,the three-dimensional reconstruction technique is developing rapidly and has been considered as the most competitive method to research pelvic artery system owing to the in vivo individualized images. There exist ethnic differences and individual variation in female pelvic arterial system. Besides the developmental changes of female pelvic artery should not be ignored. Therefore,it is of great significance to investigate the female pelvic artery system,for the knowledge of it is helpful to diagnose pelvic diseases and guide pelvic surgery and uterine artery embolization.

    Progress on Endometrial Receptivity of the Polycystic Ovary Syndrome

    The polycystic ovary syndrome(PCOS) is one of the most common endocrinopathy of reproductive-aged women in Gynecology. There is increasing evidence showing that the disorder of endometrial development and the decrease of endometrial receptivity to blastocyst implantation may be one of causes contributing to the poor abilities of reproduction and the unsatisfied outcomes of pregnancy in PCOS patients. Aberrant expression of some molecules and genes in endometrium may contribute to diminished endometrial receptivity. The article reviewed the latest progress in the well-known characteristic markers of endometrial receptivity at present,such as,the steroid hormones of ovary and their receptors,cell adhesion molecules,matrix metalloproteinase,Leukemia inhibitory factor and so on.

    Recent Progress in the Pathogenesis of Type A Insulin Resistance Syndrome

    As a rare type of autosomal recessive inheritanced insulin resistance syndrome,type A insulin resistance syndrome(TAIRS) is caused by mutations in the insulin receptor gene which induces insulin dysfunction and a series of physiological and pathological abnormalities due to defective binding of insulin to its receptors. Rarely diagnosed clinically,TAIRS is mainly characterized by severe insulin resistance,hyperinsulinemia,hyperandrogenism,and acanthosis nigricans. At present there is no explicit TAIRS diagnosis standard,and the diagnosis is mainly based on the clinical symptoms and insulin receptor gene screening. There is no radical treatment means. We just improve insulin resistance and alleviate the symptoms.

    Endometriosis is an estrogen-dependent disease. Local estrogen biosynthesis is a major factor in the pathogenesis of endometriosis. Aromatase is a key enzyme during the process of estrogen biosynthesis. Recent studies have demonstrated that aromatase both in eutopic and ectopic endometrium of patients with endometriosis expressed highly,but didn′t express in the normal endometrium,and there were several successful treatment cases of endometriosis with aromatase inhibitors. However,recent studies have found that steroidogenic factor 1(SF-1) didn′t express in normal endometrial stromal cells,but highly expressed in ectopic stromal cells. SF-1 plays an important role in the synthesis of aromatase. The level of SF-1 directly affected the expression of aromatase,while the abnormal expression of SF-1 mainly due to the hypermethylation of SF-1 gene promoter region. In this paper,we make a review about the role of SF-1 playing in the pathogenesis of endometriosis.

    Endometriosis is defined as ectopic endometrium locates outside the cavity of uterus,with pain as the most distinguish clinical manisfestation. So far,the pathogenesis of the pain in endometriosis is unclear yet. Bradykinin is the strongest algogen,and bradykinin B1 receptor is considered as one of the main factors that maintain inflammation and pain. Previous studies have revealed that bradykinin B1 receptor antagonist could effectively be used in treating inflammation and pain. Recent research progress on endometriotic pain and bradykinin B1 receptor antagonist has been reviewed in this article.

    Progress of Laparoscopic Surgery for Deep Infiltrating Endometriosis

    Surgical treatment is a main option for deep infiltrating endometriosis (DIE). Laparoscopic complete excision of endometriosis is a priority option due to a high rate of pain relief and a low rate of recurrence. However,the radical surgery of DIE leading to the dysfunction of the bladder and rectum is worthy of increasing attention. Recently,laparoscopic nerve-sparing complete excision of endometriosis is considered to reduce or avoid the dysfunction of bladder,rectum and sexuality . This article reviews the recent research progress of laparoscopic nerve-sparing complete excision of DIE.

    Sentinel lymph node(SLN) dissection has been shown to be important for staging of malignant tumor and safe for selection of alternative surgical approachs. SLN has also been shown to be associated with decreased short-term and long-term morbidity when compared with complete lymph node dissection. There has been increasing interest in the usage of SLN technology in gynecologic cancers. This review assesses the current evidence-based literature for the usage of SLN dissection in gynecologic malignancies.

    According to its mechanisms of action,gonadotrophin-releasing hormone analogs (GnRH-A) are divided into two categories:agonist and antagonist. It has been demonstrated that 80% of ovarian and endometrial cancers express GnRH and its receptors. The anti-cancer role of GnRH-A is played either by suppressing the hypothalamic-pituitary axis or through its autocrine and paracrine systems. GnRH-A had been used in the treatment of advanced or recurrent endometrial and ovarian cancers,but the effect is no better than other second-line drugs.These days,the application of GnRH-A in the preservation of fertility and ovarian function has become increasingly popular. Besides,the use of GnRH-A in molecular targeted therapy is also promising.

    Objective:To identify a set of genes related to upregulated expression of miR-205 in HeLa cells and to investigate their function,and to discuss the pathogenesis of cervical cancer. Methods:The HeLa cells upregulated expression of miR-205 and normal HeLa cells were analyzed with 23 232 human genes of cDNA microarrays. Results:A total of 172 differential genes expressed in the HeLa cells upregulated expression of miR-205 were screened out,of which,34 were up regulated and 138 were down regulated. Conclusions:The cDNA microarray is a powerful method to identify the gene expression profile. Cervical cancer induced by upregulated expression of miR-205 is a result of coeffecting by polygene and multiple factors.

    Current Situation and Thinking of Abdominal Wall Incision Endometriosis After Cesarean Section in China

    Objective:To investigate Chinese current situation of secondary abdominal wall endometriosis after cesarean section incision . Methods:In “China know nets(CNKI)” the home page,especially into the theme retrieval,input “method” and including “endometriosis”,select “accurate”,and click “retrieval literature”,ideally suited to learn about “after cesarean section abdominal incision endometriosis” articles,click “the author unit” of the province/municipality and “published time” in the annual statistical analysis. Results:① in CNKI,“after cesarean section abdominal incision endometriosis” literature is a total of 427 article,total 5 169 patients,“the author unit” of the province/involved were 29,85.29%(29/34) municipality were involved. The top five provinces is Jiangsu,Zhejiang,Henan,Shandong and Sichuan provinces,the last five is mingier,Fujian,Jiangxi,Gansu and Qinghai provinces. ② in CNKI,about “after cesarean section abdominal incision endometriosis” aspects of the article,first published in 1984,there were 70 articles in earlier 20 years,accounted for 16.39%. After 2004,the incidence has increased year by year,then reached to 18.03% in 2010. Conclusions:①After cesarean section abdominal incision endometriosis, the pathogenic factors support endometrial “planting theory”. ②The abdominal incisions endometriosis after cesarean section displays obviously regional characteristic,probably closely related with its “model” section. ③The cesarean section postoperative abdominal incision endometriosis increases year by year. ④The improvement of “cesarean section postoperative abdominal incision endometriosis”awareness,strict control of cesarean section indications,control the rate of cesarean section,enhance the quality of operation,reduce abdominal incision endometriosis key initiatives,perhaps also key to reduce or eliminate the medical disputes in the future.

    Objective:To investigate and analyze the nutrition knowledge,attitude and practice among pregnant women,for scientific evidence of better grasp of nutrition knowledge and more targered nutrition guide. Methods:392 pregnant women in Yellow River Central Hospital were selected randomly to investigate their nutrition knowledge,attitude,practice (KAP) by a self-designed questionnaire. Data were inputed using Epidata 3.1. Statistical analyses were performed using SPSS 13.0. Results:Most pregnant women lacked a overall understanding of nutritional knowledge and practice,however,they held a positive attitude towards nutrition. They hoped to obtain more professional knowledge from medical staff. Conclusions:Doctors and nurses should improve the nutritional level of knowledge,strengthen health care for pregnant women,and to improve pregnant women nutrition knowledge and attitudes and to change its unhealthy eating behaviors,and improve mother and children′s health.

    Clinical Value and Ultrasound Screening of Fetal Single Umbilical Artery

    Objective:To evaluate the clinical role and the ultrasonographic features in prenatal diagnosis of single umbilical artery(SUA) in Tianjin. Methods:To review 40 fetuses with SUA found in our hospital from December 2007 to July
    2009 in Tianjin. Their clinical manifestation and pregnancy outcome were followed up. Results:In 40 cases,only single umbilical artery could be detected by ultrasonography. Among them,5 cases combined with other abnormalities by ultrasonography,1 case was induced of labor because of intrauterine fetal death. In the cases going on with their pregnancy,12 cases had disorders during the pregnancy course or the labor,7 cases were found to be combined with other abnormalities. Conclusions:Ultrasound-Doppler can diagnose the SUA correctly. When we find a fetus with SUA,we should analyze it from different aspect to except other organic malformations. To the cases going on with their pregnancy,we must pay more attention to the fetuses growth and delivery.

    Clinical Analysis of Diagnosis and Treatment for 66 Cases of Acute Appendicitis During Pregnancy

    Objective:To explore diagnosis and treatment of acute appendicitis during pregnancy. Methods:retrospective analysis of the clinical data of 66 cases of acute appendicitis during pregnancy,29 patients didn't agree with surgery,so received conservative treatment,and 12 cases of uncontrolled infection,change to surgical treatment,37 cases were admitted to the hospital emergency surgical treatment. Results:66 patients discharge smoothly;eight patients after discharge required abortion in obstetrics and gynecology;2 patients had the abortion,one patient was premature birth at 10 days after surgery;no serious complications such as fetal distress,stillbirth and pregnant women death. Conclusions:Important means for the safety of maternal and infant is that timely diagnosis and surgical treatment of acute appendicitis during pregnancy. If symptoms of light infection,unapparent local peritonitis,and patients don't agree with surgery,we first do conservative treatment,and closely observe. If the treatment effect is not obvious,we then immediately surgery.

    Objective:To study the ability of the No.1 and No.2 of type Ⅱ of Mefordcare Phytic Acid Ketone on
    inhibiting the proliferation of HeLa cells which were cultivated in vitro,and provide the evidence for futher experiment. Methods:The HeLa cells which were cultivated in vitro was treated by eight different concentrations for 24,48 and 72h,MTT assay was employed to examine the growth suppression of HeLa cells. Results:Contrast with the control group,the apoptosis of the cells treated with these two numbers of the drug was increased and the growth rate was decreased. And the inhibition of HeLa cells in partly is in a concentration-dependent and time-dependent manner. Conclusions:The No.1 and No.2 of type Ⅱ of Mefordcare Phytic Acid Ketone could significantly inhibit the proliferation of HeLa cells.
